Wisconsin.  The county trunk highways (CTHs) are always on BGSs.

Examples (of which there are very, very many):

I was in the mood for some overhead signage from Wisconsin.

How about where the county highway is the only destination on the sign?

Iowa, FTW!

Seriously, that's an awful sign.

The White Pole Road exit, CR G30. It's a historic section of US 6 while US 6 currently follows I-80 much of the way through Iowa. That section of the White Pole Road southwest of here is also particularly scenic and pleasant.

On the Indiana Toll Road, the signs for Exit 96 include the blue county highway shield for County Road 17 (even though the road itself never uses that shield).
Indiana: counties 100%, highways 100%
Illinois: counties 100%, highways 61%
Michigan: counties 100%, highways 56%
Wisconsin: counties 86%, highways 23%

And the "destination" is "Elkhart East".

Minnesota, very much.

It's funny in the two cases shields have been seemingly arbitrarily greened out presumably to avoid confusion with other routes: I-494 at CR 5 in Minnetonka (to avoid confusion with MN 5 five miles away), and I-494 at CR 52 in Bloomington (to avoid confusion with US 52, I assume, which is about ten miles away). Yet the CR 35 shields on the Portland Avenue BGSs were kept despite being three miles from I-35W.

Also, good luck finding an actual county highway shield on a BGS near the interchange of I-35W and US/County-10 north of Minneapolis.  The absence is so striking, it must be to avoid confusion.
Keep right except to pass.  Yes.  You.
Visit scenic Orleans County, NY!

????

(GMSV from 2015 shows it's still there, as is a CSAH 10 shield for the northbound off-ramp)

Now, if you're referring to county routes other than CSAH 10, it has to do with Ramsey County generally not putting an emphasis on it's numbered county routes, with the exception of those routes that are decommissioned state highway segments (such as CSAH 10, CSAH 49, and CSAH 96).  Heck, until about 15-20 years ago, Ramsey County didn't even sign their CSAH route numbers.  Anywhere.

New Mexico which, as we know, has no affinity for anything regarding convention when it comes to signage, treats drivers to this haphazard homage to TxDOT's F.M./R.M. BGS convention on I-25 south of Santa Fe.

This example is, possibly, the only pentagonal county shield in the state of Ohio on a BGS.

Are there others? If so, I have not encountered them yet.

The only time I've seen it is on construction BOSs. In advance of a contraflow near I-75 Exit 78 this year, there was signage "TO [Pentagon Miami County 25A] RIGHT LANE". Miami County doesn't use pentagons. This same area had detour assemblies with black-on-white pentagon shields. Contractors sure do interesting things sometimes.

But no, I can't think of anywhere in Ohio where I've seen a county road in shield form on a BGS, pentagonal, square, or otherwise.

Illinois has a few here and there, but they are rare.

It's super-rare in Illinois, but there are two other downstate examples I can add to the mix:

And I thought I saw a McDonough County 8 BGS along IL-336 someone posted recently in another thread

Iowa uses them almost everywhere; there's a whole network of them.

For some reason Iowa does not seem to sign them as much on I-80 east of Des Moines though - there are several interchanges where only the nearby towns are listed and the county route is omitted from the sign.

Taney County 165 being signed for an exit of US 65 in Branson may be the only occurrence of this in Missouri:  Streetview

Outside of Suffolk County, NY where are County routes signed or written out on BGS?

How about Rockland County, NY?  Lots of photos available at http://www.greaternyroads.info/roads/nypark/palisade and http://www.greaternyroads.info/roads/nystate/ny59.
