Non Interstate Service Plazas?

Not really a "Service Plaza" (those don't really exist out west), but this is probably the closest thing to one out here, interstate or not. It's on US-95 in Idaho. It's signed as a "Rest Area" with the normal blue signage, but it's actually basically just a gas station. I didn't stop there myself so I'm not sure if they have any additional facilities, but it looked just like an Exxon station/C-Store driving by it. Unfortunately, GSV is too old and blurry in the area to get a better look.

Here is what ITD's website says about it:

This rest area at milepost 278 has restrooms, drinking fountains, vending machines and a telephone. It is located at the junction of Winchester and U.S. 95 at Camas Express. This is a partnership between Camas Express, Nez Perce Tribe and ITD. Rest area users are not required to purchase items from Camas Express.

Most service plazas around where I've been are on toll roads regardless of whether they are interstates. Obviously some listed in this thread are not toll roads, and there's the CT Turnpike which is no longer tolled (but is I-95).

However, outside the U.S., service plazas similar to those mostly found on U.S. toll roads are quite common, even on "free" roads. 

Ontario has Highway Service Centres along Highways 400 and 401.  Quebec has service plazas called Aire de service along several Autoroute corridors.

Germany has them on its "free" [for passenger cars] Autobahn network. The Nordic nations have them on their motorway networks too.

The merit parkway has a good number. I think there is also one on Mass 24.

What about the psudeo-service plaza at exit 6 on the Mid-Cape?  Or does that not count because its accessible from MA 134?

That's equivalent to getting off an exit and going to a store there, so no.

Already mentioned; see Reply #11 on Page 1 of this thread.  There's actually two of them; one on each side of MA 24.
