There are two in Indiana that are one number off: SRs 218 and 261. SR 218 isn't in AC 219, though, and SR 261 and AC 260 are in opposite corners of the state.

MN 218 once was in AC 218, but hasn't extended that far since the early 1960s.

I would assume that New Jersey must have a CR 609 somewhere in the 609 area code.  Whether it's in a county that signs its routes is an entirely different matter, however.  There's also a CR 551 in the state, but in the entirely opposite corner from the 201/551 area code.
There's certainly a 609 in the 609. I wasn't really looking for counties and other secondaries - though it's valid!

Can't think of any in Florida, but there is an sign on I-95 south that says Miami 305 (miles)...photo on laptop, sorry.

305 wouldn't be in Miami-Made or Monroe Counties.
954 nor 754 wouldn't be in Broward County, although it would be close by, on the grid.
SR 786 is in Palm Beach County, but outside its area code.
863 doesn't exist (I think), but it could sort of fit in its area code if it did.
813, 850, 727, 904, wouldn't be that far north.
352 and 321 are mysteries to me, I'm not sure they exist, but I could be wrong...they would be out of the grid, anyhow.

Arkansas: Area Codes 501, 479, 870...No match

Missouri: Area Codes 417, 314, 636, 573, 660, 816...No match

Kind of surprising that MO doesn't have a match on at least 417... I know there is a 413 in area code 417, but that of course is numbered so because it is old 13.

Oklahoma's area codes are 405, 580, and 918. Oklahoma's highest state highway number is 325, so...

Louisiana only has one match. LA 337 near St. Martinville/Lafayette.  LA 318 may be a historic one. It's near Jeanerette, but I can't recall if 318 ever extended that far south. It was either 318 or 504.

The 337 area code is the former southern section of the 318 area code. 504 used to encompass all areas east of the Atchafalaya River until the 225 and 985 was carved out back in the 90s.

LA 985 is in the 225 and LA 504 is in the 318.

Until 1999, however, SR 219 existed in AC 219.

NY 315 is in Area Code 315.  NY 315 can be found south of Utica, connecting NY 12B with NY 12.

While PA 570 was decommissioned in 1946, its routing is in present day Area Code 570.

I forgot about 561 (that is, until I passed by on the Turnpike yesterday) which seems to be a county road in Lake County, roughly 200 miles NW of area code 561.

Forgot about 407, although the area code was created in the 1980s, it's rapidly shrunken in area since then.

The only area code in North Carolina with a corresponding route number is 704, but NC 704 is currently entirely within area code 336. Historically, all of North Carolina was area code 704. Until the split with 919 in 1954, NC 704 was within area code 704. No other area code in NC has been used as a route number in NC.

None of MS's four area codes (228, 601, 662, 769) have matching highways. The closest would've been MS 603 in Hancock County, when that part of the state was in 601 (it's been in 228 since 1998).

There are several ACs in Pennsylvania with corresponding SRs, but none are grouped together:

412 (Pittsburgh) - PA 412 (Bucks and Northampton Counties)
724 (Pittsburgh suburbs) - PA 724 (Berks and Chester Counties)
215 (Philadelphia) - PA 215 (Erie County)
484 (Philadelphia suburbs) - PA 484 (Fulton County)
267 (Philadelphia suburbs) - PA 267 (Wyoming and Susquehanna Counties)
