The first one is Woodward Avenue in Detroit, 1942.

My dad said the "Death Here" sign is because someone actually did die there in an accident.  It is much how people put up crosses around here for the same reason.

That seems very plausible.  I faintly remember that the causeway had a place to turn around somewhere in the middle.  The picture would also have to predate the parallel span.

The only other plausible place that I can think of is the chesapeake (sp?) bay bridge tunnel

'tis, a bit south of the midpoint. You can actually see the parallel span in the background.
