Thought of a couple of other "San Francisco" examples:

Route 24 west just past the Caldecott Tunnel in the Oakland hills
US 97 south in Klamath Falls, Oregon (one of the more unusual places to find it!)

I am not sure does anyone know about I-39 in WI and Rockford and US 12 in WI have Chicago?
The 39 SB control city is Madison.  South of Madison, Chicago is used.  But Rockford also makes many appearances.
As for US 12 in Walworth County, I recall Lake Geneva being used as a control city.
One slight correction.  When you enter the US 12 Eastbound freeway from WI 67, the sign says Chicago and Lake Geneva via freeway.  At the I-43 junction, Chicago is not mentioned.

Also i would say Madison may have a fair amount. 
I-90 E/W (WB starting at I-294, EB at La Crosse)
I-94 E/W (WB in Milwaukee, EB in St Paul)
I-39 N/S
US 12 W (at I-43)
US 14 W (in Fitchburg WI)
US 151 N/S
US 51 S (kind of a stretch, but there is an exit at WI 19)
US 18 E/W

67 and IL 155 should be St Louis but I think they are Alton

I-155 SB uses Lincoln, but I recall Decatur making an appearance on one of the distance signs; St. Louis doesn't pop up for US 67 until the entrance to the Clark Bridge.
St. Louis also appears as a control city for the US 67/167 freeway at I-40 in Arkansas.

In Connecticut it is signed as a control city on:
I-91 SB south of Hartford (along with New Haven)
I-95 SB
The Wilbur Cross and Merritt Parkways SB (CT-15)


Let's not forget that I-95 NB is signed "New York" heading north out of Baltimore (there is no mention of Philly until the 295 split in DE.   Along those lines, I-895 NB out of Baltimore (which merges into 95) also lists NY as the control city.  Interesting that they list NY, but not Philly, and 95 technically doesn't even go directly to NY (thanks to the NJ gap).

Less we forget the NJ Turnpike NB is signed New York for its entire route.

I-80 EB is signed for NY at the Ohio Turnpike split.

The Garden State Parkway NB has NEw York as the control city, at least north of Atlantic City.

It may not be the primary city, but the NY Thruway (I-90 EB) counts down mileage to NYC as you travel eastbound from PA/Ripley.   The control cities are Buffalo then Albany, but the mileage signs (which used to be posted every 10 miles at 0 intervals - 490, 480, 470...) included mileage to the NYC line.  This is a bit ironic, since most mileage to a city is measured to the center or business district, and since the Thruway 0 marker is at the Bronx line.   As you travel east then south, the mileage counts down, so you really always know how far you are from the NYC line.

Plus while not the primary control city, distances to Toronto also appear on directional and distance signs on the freeway sections of King's Highways 8 and 402.
I'm not aware of any references to it in Michigan or NY State as they tend to simply reference the Canadian border from what I recall.
Have seen signs referencing Montréal in Albany, though.

Hawkinsville, GA is used on many exit ramp guides between Macon and Cordele as many roads lead there.
Arcacdia, FL has many exits for it on I-75 from Bradenton to Punta Gorda.  Many roads lead there, but FL 72 merges into FL 70.

I was in Bakersfield recently and I thought it was curious that the control city for CA 184 was signed for Los Angeles.  And again at the 184/223 intersection near Lamont.

Also, 505 South is also signed as headed to San Francisco.  And I don't know if the 980 EAST San Francisco sign is still up in Downtown Oakland.

New York (without "City") is signed at multiple locations on I-95 south in Rhode Island as well.

Where is 805 north signed for San Diego?  Does 94 west have any signage for San Diego?

New York makes an appearance in downtown Boston as a control city for the Mass Pike westbound.  Steve's page has a picture, button copy and allFurther west, some signs say "Worcester/Springfield" while others say "Worcester/New York", including that fun one with a small-caps T in WorcesTer.  It's not clear whether they mean New York City or State, but I suspect City (via I-84).

In Shreveport you also have Dallas as control city on I-220 WB and LA 3132 WB. And I'm pretty sure AR 245/ TX 151 in Texarkana too. For sure where Future I-49/AR 549 meets AR 245 from the south

I'd suspect Los Angeles would be the most used control city in America! After all, it's the city that "invented the freeway", so to speak.
