Delaware 286 was commissioned in 1993 to provide continuity with Maryland 286 to South Chesapeake City. The state route extends Bethel Church Road west from the southward turn of Delaware 15 onto Choptank Road. A trio of large lot subdivisions line the otherwise rural route to Maryland 286 (Bethel Road).
The intersection of SR 15 and SR 286 was converted to a roundabout as part of a three-year $11-million reconstruction of Choptank Road. The roundabout on Bethel Church Road was completed on November 20, 2009, while the remainder of work on Choptank Road south wrapped up on May 27, 2010.1,2
