U.S. 422 is one of a few U.S. routes that have two disjointed sections (others include U.S. 2 and U.S. 10). The western segment runs east from Youngstown, Ohio to New Castle, Kittanning, Indiana and the freeway along U.S. 219 at Ebensburg. U.S. 422 reemerges east from U.S. 322 and PA 39 at Hummelstown. The eastern segment travels to Lebanon, Reading, Pottstown and King of Prussia outside Philadelphia.

Opened on December 20, 1963, the freeway taking U.S. 422 around Reading was constructed at a cost of $11 million. The 7.1 mile long freeway stretched eastward from the Warren Street Bypass (PA 12) to Philadelphia Pike (Perkiomen Avenue) near 47th Street in Reiffton.1

The east end of U.S. 422 is with U.S. 202 south of the Pennsylvania Turnpike and west of Interstate 76 at King of Prussia. A major reconstruction project from 2001 to 2003 rebuilt U.S. 202 and the exchanges with both I-76 (Schuylkill Expressway) and U.S. 422. Work also rebuilt the final mile of U.S. 422, and added c/d roadways along U.S. 202 from U.S. 422 to I-76.

U.S. 422 previously continued southeast to PA 309 (former U.S. 309) in Philadelphia. Originally the route extended further to the Tacony-Palmyra Bridge and the New Jersey state line at the Delaware River. Upon completion of the Schuylkill Expressway Extension from Pottstown to King of Prussia, U.S. 422 was relocated from Germantown Pike and truncated westward.
