U.S. 20 crosses the width of the Empire State from the Pennsylvania state line at Ripley to Massachusetts from the town of New Lebanon. Northeast along Lake Erie through Westfield, Fredonia and Silver Creek, U.S. 20 parallels the New York Thruway (I-90). The two diverge in the Buffalo metropolitan area at Hamburg, where U.S. 20A also begins. U.S. 20 turns east at Lancaster, continuing to Aiden and into rural Genesee County.

U.S. 20 travels well south of the Rochester area, passing through the town of Avon and villages of Lima and Bloomfield. Through the Finger Lakes Region, U.S. 20 expands to four lanes through the cities of Canandaigua, Geneva and Auburn. Further east, U.S. 20 shifts southward to avoid the Syracuse metropolitan area. The highway remains rural into Madison and Herkimer Counties, running well outside the Utica area.

Approaching the Capital District south of Schenectady, U.S. 20 crosses Interstate 88 in the town of Duanesburg. Entering Albany County, U.S. 20 becomes more suburban through the town of Guilderland. The US highway converges with the New York Thruway again by Crossgates Mall and the University of Albany. East from there, U.S. 20 crosses the city of Albany, combining with U.S. 9 from Downtown along the Dunn Memorial Bridge to Rensselaer.
