In Vestal, NY (a suburb of Binghamton), the main street is called the Vestal Parkway (NY 434, former NY 17). Before the parkway was built, the main road through town was Vestal Rd. (also former NY 17).

Close to that, in the town of Conklin, the main road is Conklin Rd (NY 7).

This might be less applicable, but the road through the hamlet of Union Center is called the Union Center Maine Hwy (NY 26).

Some Chicagoland examples:

The main street through La Grange, IL, is La Grange Road (carries US Routes 12, 20, and 45).

The main east-west street crossing the river in Algonquin, IL, is Algonquin Road.

The main east-west street in Palatine is Palatine Road.

Cicero Avenue is certainly the most heavily-traveled north-south thoroughfare in Cicero, though it isn't centrally located.

In Illinois Monmouth street is US 67 in Good Hope and does go to Monmouth where it's used to be Main Street and now is the Monmouth bypass which is now busier than the nominal Main Street.
There is also a Monmouth Blvd in Galesburg that has never connected to the nearby town of Monmouth and used to be a major street but no longer is.

The street through downtown Mena, Arkansas is called Mena Street. Part of it is AR 88 and part of it is old AR 8. The locals call it "Main Street". I don't remember anybody calling it Mena Street when I was growing up very often.

Another one in West Virginia:  The main street in Winfield is called Winfield Road, which runs all the way from US-60 in Kanawha County to the Mason County Line.

US 69 in Ankeny, IA is Ankeny Blvd.

And IA 141 through Grimes is Grimes Boulevard.

Clive's main thoroughfare was known as Clive Road at one point, but it's now known as 86th Street with "Historic Clive Road" signage posted.

Urbandale has an Urbandale Avenue, but I wouldn't call it a "main thoroughfare"; it's a westward continuation of the street of the same name in Des Moines. It curves northward and turns into 70th Street just after entering the Urbandale city limits.

Kenilworth, NJ has Kenilworth Blvd. However, the locals don’t call it that. Just “ The Boulevard “ or just “ Boulevard.”

The street blades just use the latter, but only maps refer to it by its proper name.

Most of Front Street in Wilmington was souped up and redesignated as Wilmington Blvd in the late 1970s or so, but in 1987 was renamed MLK Blvd.

Salisbury, Maryland's main thoroughfares are Salisbury Blvd. and Salisbury Parkway, but at least part of its older Main Street still exists.  Salisbury Blvd. becomes Fruitland Blvd. as US 13 Business crosses from Salisbury into Fruitland.

There is an Elsmere Blvd but it's not really a major street and half of it is in Wilmington.

In Chesterfield County, there is:

- Chester Road (VA 144 and VA 145), it goes through the center of Chester, and carries many neighborhoods, however it does not go through "downtown" .
- Midlothian Turnpike (US 60), it goes.. well through Midlothian. Theres actually not much to say about this highway.
- Meadowville Road (SR 618), it goes through the residential areas of Enon (it is actually still under construction)

- Richmond Highway (US 1 and US 301), it is one of the replacements of Jefferson Davis Highway. Fun fact: there are portions of Richmond Highway in Northern Virginia.
- Mechanicsville Turnpike (US 360?)
- I think I've heard Fredericksburg Turnpike? (My guess is VA 2)

St George Blvd (SR 34, former US 91) is the main drag through downtown St George
Riverdale Rd (SR 26, former US 91) is the biggest road in Riverdale
Vernal has Vernal Avenue (US 191 north of the center) as their north-south axis street
Hyde Park Lane is the east-west axis for Hyde Park and the main way to get there from US 91
Tremont Street, while not a particularly important road, is the north-south axis street for Tremonton

In the Rochester, NY area there are:
Chili Ave (NY 33A) which is the main road that runs through the town.
Penfield Rd. (NY 441) which is the main road that runs through the town and is also one of the busier roads in Monroe County.
Webster Rd. (NY 250) which is the main N/S road through the Village of Webster.

West and East Henrietta Rds. (NY 15 and 15A) were mentioned upthread.

Commerce Township, (Oakland Co) Michigan has Commerce Rd, E Commerce Rd and S Commerce Rd.

S Commerce Rd runs n/s.

Commerce Rd runs e/w, east of S Commerce.

E Commerce Rd also runs e/w, west of S Commerce, on a line 1/2 mile north of Commerce Rd.
