I'd be happy to retract this if someone has one with actual shields, but here's Virginia Secondary 669 - US 11 = 658.  I-81 in West Virginia, even though it's for an exit over the border in Virginia.  December 26, 2010.

Jim, I tried several of the above examples, but none would show up in image searches. Only one had both signs in view with Street View, and even then...this intersection of VA 8 and VA 669 (difference of 661) is a bit questionable:
