Curious if this happens anywhere? Examples with "TO" above one or more directions will count.

On the same physical panel, or the entire collection on a gantry?

This is separate panels but same gantry. Crediting billburmaster.com and his page on IL I-94

I have a feeling this will be difficult to find. I think we've established over the years that this doesn't exist on a single highway with all 4 directions.

If it exists on one sign panel it's likely to be one that has A-B suffixes and splits at the exit gore to both directions.

Happens now entering the Indy Beltway/I-465/69/74 on the southern leg, since 465 and 74 are signed E-W and 69 is signed N-S

The first picture is at the ramp from southbound I-65 to the I-465 ramps. It is interesting how there are three control cities along the Ohio River all in this one location.

The second picture is at the split of the westbound and eastbound ramps. Coming from the south, the northbound I-65 ramp to I-465 has a similar set of signs.

Another example on a single BGS panel with all four cardinal directions is East St Louis, at the I-55/64/70 interchange

I-55S/I-70W: Left Exit 3A I-64 East/IL 3 North | I-64/US 40 West/I-55/IL 3/Great River Road South

I was hoping for ones on the same physical panel.  I'll settle for ones with a dividing line as others have posted.
