Almost every New York state highway at a county border has the county and then the town or city.  At the state line, you can get the state added in as well, so I'll go with one of those entering New York, and a similar situation leaving New York.

Vermont 346 as it becomes New York 346.  Back on December 26, 2000.



Entering Pennsylvania as New York 5 becomes Pennsylvania 5.  August 9, 2002.

College Ave and Washington Ave in Des Moines, extra appropriate because of the other street name :)

I know what you meant, but I consider the hockey tournament the real "March Madness", not that squeaky, overhyped other game.  With the 2014 national champs Union College back in the field of 16 this year, I offer this from US 34 in Nebraska.  July 28, 2016.
