The closest to me is another Texas route, FM-925 about 1,500 miles away.  Other farther options are in Florida, Louisiana, Maryland, Pennsylvania, and Puerto Rico.  If I'm willing to go international, there is also one is Saskatchewan.

Mine would be OH 515, some 625 miles from home.

IA 319, which was decommissioned in 1980, existed near the town of Gravity in southwest Iowa, which is actually in the 712 area code (319 serves eastern Iowa). Iowa never had any state highways numbered 515, 563, 641, or 712.

It's going to be State Route 515 in Ohio at around 700 miles.
  • Alberta Highway 515 is nearly 1300 miles
  • Interstate 515 (for now) is about 1400 miles

I live in the 215 area code in southeast Pennsylvania, the closest Route 215 to here is in Virginia. I also have 267 and 445 as overlay area codes. The closest routes with those numbers are in Maryland.

For the 714 area code, it’s Texas FM 714 near Brady, approximately 1,090 miles away.
For the 657 overlay, it’s Texas FM 657 near Santa Anna, TX (not Santa Ana, CA), approximately 1,100 miles away.
For the 949 area code used on my only telephone, it’s Texas FM 949 near Eagle Lake, approximately 1,300 miles away.

The runners up respectively appear to be in Centerville, OH, Riceville, LA, and Penrod, KY.

Well, Farm to Market roads sure are our savior.

Closest, and seemingly the only 919 highway is FM 919, in Palo Pinto County. 919 however is overlayed with 984, and MD 984, an unsigned service road in Ellicott City, is significantly closer.

I assume it's either NY 418 or ON 418. As for overlay code 581, it's likely either PA 581 or ON 581.

As I've moved, I've gotten new numbers so my area code matches my location.

I used to do that, but I stopped, coincidentally enough in 2005.

I already did 816 as the OP, and this is post number 913 for me, corresponding to the other area code in the KC metro (the one on the Kansas side), so I figured I would do that one as well!

Anyway, the closest road to me with that number appears to be KY 913, a short road on the east side of Louisville - about 490 miles straight-line distance from KC.

Any Route 856s within 1,000 miles of South Jersey appear to be small, minor roads, maybe in Virginia or the Carolinas.

My former area code before it was split in the 1990's, 609, would've featured at least two County Route 609 within the area code itself, since 6xx routes can repeat throughout the state.

Well, I have to answer this in different ways. I didn't change my phone number when I moved, so I currently have a 913 number. But, I'm in the 303/720 area, so:

303 - UT303 which is just southwest of Green River
720 - NV720 way out by Fallon
913 - KY913 outside of Louisville
I'm hosed here in 970, unless there's some FM or forest road somewhere.

Well, perhaps you should petition CDOT to designate one of the Denver-area freeways - either the Denver-Boulder Turnpike or Peña Boulevard (the freeway leading to the airport) - as I-970. Either road would be an ideal candidate for that number, in my opinion - the other road could be either I-570 or I-770.

Funnily enough the area code of my cell phone doesn't match where I've ever lived exactly, at least since the number was issued. By the time I got cell service 856 was separated from 609, but for some reason they went with 609 anyway, and while I've lived in 732 and 973 since then, before moving back to 856, I've never lived in 609.
