In my experience when it comes to reporting issues or suggesting maintenance, TDOT has been the most responsive. I have submitted three comments to them: One for remarking the guidelines in an intersection, one for trimming trees for visibility, and one for adding markings through a curved intersection where it's difficult to see at night. Of all of those, I have received a detailed response and been notified that my complaint was added to the maintenance schedule.

PennDOT has also been very responsive in the single time I've reported something to them. I noticed some pretty massive concrete chunks just laying on a parking lot underneath one of the expressways in Pittsburgh, and let them know that it was possible they could have fell from the bridge above. They updated me with several emails and sent someone out to check on it and let me know everything looked good.

On the other hand, I have not always had luck with getting a detailed response from NCDOT. I have had one (potential) success though -  I reported a blind intersection on a curve out in the middle of nowhere, and commented that I pretty much had to take my chances whenever I turned left, and a few months later, it was changed into an all-way stop. I never heard back from them about it though, so I can't be sure that it was my feedback that led to that decision.

At least in NYSDOT Region 3, there's a policy to respond to every public inquiry.  It may not happen quickly, but a response is sent back.
