There is a couple with my real last name in Virginia, Pennsylvania and Maryland. No shock considering that is where most of Dad's side of the family settled from England.

Makes sense. There are a few streets with my last name in my hometown, and this is true of many families there.
Both my last name and my wife's maiden name are street names one block apart in Trenton.

So there is a street there with a sign blade that reads "D AVE", which my childhood self thought was pretty neat.

Is that style of street sign blade common in Arkansas? I used to be a regular on the Howard Stern subreddit and found/posted a street in Searcy, AR with the name "Scott Salem Road," sharing the name with the show's Scott the Engineer (kch). That sign was the same type of yellow on green text.
Re:thread, no road names in my area with my full or even last name, probably a lot of Matthew/Matt streets around though in McMansion developments. My professional engineer license number however is a concatenation of two South Jersey freeways' numbers though.

Vancouver, BC, has separate streets bearing my first name in plural form (that is, my first name with an "s" on the end), my middle name, and my last name. I was rather struck by that when I noticed it on street signs when we visited the city.
My father grew up three blocks from a street in Brooklyn with our family's last name. Perhaps more bizarrely, his first year at UVA Law he lived in an on-Grounds residence with that name. (Didn't live there the next two years because he and my mom got married and lived off-Grounds.) Some 23 years later, I had the chance to live in the same building but passed in favor of a place with better access to parking.

There's a Patrick Street in Alexandria, Virginia. It's a paired one-way street with Henry Street. The two go together (making up US-1) in honor of colonial leader Patrick Henry.

I haven't checked if there are any roads with my given name, but I'm pretty aware of a "city" (more of a village actually) in Minnesota.

My name is Frank Foster, and I found two Frank Foster Roads - one in Merryville, LA, and one in Macon, GA. Both are fairly short roads. There was a fairly famous jazz musician named Frank Foster, for whom one or both of these could have been named. Interestingly, he was originally from Cincinnati, too.
There's a Frank Rd in Columbus (OH-104, exit 104 off I-71).
My hometown of Cincinnati does have both a Frank St and a Foster St, but they are both only 1 block long, and several miles apart. There is also a small town (crossroads) about 25 miles away named Foster, and there are a couple roads leading to it with Foster in the name (Socialville-Foster Rd, and Foster-Maineville Rd.)
I looked to see if there's any place where there is an intersection of Frank and Foster, and the only one I could find was in Eltham, Victoria, Australia, a suburb of Melbourne. Unfortunately, the only street sign in Google street view is a single blade of just Foster Rd.

My brother, in the last year, sent me a picture of a sign with my mother's family name, a name that I don't ever use except in Latin America.
I thought that I had tripped over a short street with the Piemontese-Italian spelling that I use of my father's family name in Western Italy or Eastern France, possibly near where my great uncle had his church (like, it might just be the name given to the long driveway in front of the church), but, when I looked for it yesterday, I wasn't able to find it. I was able to find at least half a dozen streets in the Francophonic world which use the French spelling of my family name.
Probably one-third of the cities in the Anglophonic world have a street with my wife's father's family name. California even has a city with my wife's mother's family name. A variant spelling is even a fairly large city in California.
My Ex- has a whole region in Italy with her father's family name (and a variant spelling for a whole island in the Caribbean) and even a fairly large city in California with her mother's family name.

There's a fairly short road in the Piemonte near where my great uncle was monsignor with my family name, "Rua Ravera".
I suspect that there are hundreds of roads named "Michael" or "Angelo" or "Miguel" or "Angel" or some combination of these. I think that the traffic circle in front of the mission in California is something like "San Miguel Arcangelo road".

I actually drive on a road with my first name somewhat regularly. It's not meant as a through road, but I use it as an alternate to bypass a congested intersection, so fittingly, it does serve a purpose that's somewhat specific to me!

I have such a weird surname that the only way a road will have that name is if I get to name it.
With my username being Hunty2022, it's pretty obvious what my actual name is.
The closest Hunter Road to me appears to be VA SR-800 south of Staunton.
I wasn't able to find a Hunty Road.

We discovered a Taber Street in Williams, AZ on our way to the Grand Canyon. (I-40 wasn't built around Williams yet - this was 1980.)

There is a street with the same name as me in the town I grew up in. It's somewhat common as a street name since it was the last name of one of the presidents.
Anyway, when I first got my driver's license, my father, who had accompanied me, did not let me drive home from the DMV. Rather, he took me this street, and then let me drive. So, the first street I drove on as an officially licensed driver was the street with my name. A few years later, they replaced the street blades, and I managed to get one of the old ones, which is now in my bedroom.
