True, but after the FHWA rescinded the interim approval the first time, several Clearview states moved away from it. Notable examples include Arizona, Ohio, and Illinois. So even though some states still use it religiously (Texas and Michigan), I feel like it's already less common than it once was. This could always change again though.

Virginia started using it again as soon as it was available again.

I think it's probably mostly because the people who don't like it know their side has effectively "won". When it doesn't seem to be the big change coming down the pipe anymore, there's less of a need for you to point out how much it sucks. FHWA does that for you now.

I misread this at first until I realized you were actually suggesting two separate warnings. I'm in favor of both of these warnings, if possible. This would help cut down on the massive quote strings and misattributed quotes.

It wouldn't solve people typing in the middle of what they're quoting, but maybe there could be a warning for that as well, something like: "You added text to an existing quote!" (Of course, this would also come up in cases where it was intentional, like the (1) and (2) that I just added to this quote.)

Not something the forum software allows us to do.

Some other forum software packages actually start automatically deleting more than one or two levels of quote.

I'm just glad that people actually quote instead of just spewing their thoughts out and expect you to do the work to understand what they're referring to.
