In Middletown, NY, there is this overhead BGS on N Galleria Dr approaching NY-211. Here, NY-17 is upgraded to an eastern version of I-17 (they jumped the gun on the interstate designation and got the number wrong), and two new suffixed US Routes are added: US-211E and US-211W.

That's one of the most egregious clusters I've seen.  Someone really screwed up.
2di's clinched: 44, 66, 68, 71, 72, 74, 78, 83, 84(east), 86(east), 88(east), 96

Agreed.

This one definitely belongs in the Worst Of thread. It's just beyond fucked up.

This is the closest thread I can find for this.  It isn't in the USA, and it isn't quite a federal/state mixup shield error.  But it's still pretty close in spirit.

Last Thursday, I saw this sign on Av. Morones Prieto in Monterrey (Nuevo León, Mexico) while heading out of town.

Some context for the error:
Av. Morones Prieto is Eje Metropolitano (Metropolitan Trunk Road, which is a Monterrey-only system) 20, so the blue shields on the signs are correct.  However, it turns into Carretera Federal (Federal Highway) 40D, so the white shield is an error.  As far as I know, there is no such thing as Carretera Federal 20 anywhere in Mexico.  Other sign gantries along the route get it right;  this was the only one I saw that had the wrong number.

Keep right except to pass.  Yes.  You.

Here's a happy error. Just west of Depew, OK.

SM-G892A

My suspicion: they posted that instead of posting the "Historic US 66"  shield in addition to the OK 66 shield. Oklahoma has been signing sections of the old 66.

ARDOT has made this error in a number of locales. This is Flippin, AR

near Joplin, MO

Should be AR 54

This one still lives after several years in Galveston, TX.
Every day is a winding road, you just got to get used to it.

They tried.  From further back especially, you can see that the middle lane has the painted arrow at an angle to correspond to the "bear left" of the overhead (which is like a usual left turn lane assignment sign), while the left lane has the regular turn arrow painted and the overhead has a much more bent arrow (the kind used with directional assemblies) vs. the one usually seen on a turn lane assignment with more curvature.
It would still be clearer if they painted an arrow with some kind of slight bend to it, despite it being a custom job vs. something with a template.

At that point, might as well turn it into a multi-state route 202, just like MSR 200.  :bigass:

ARDOT doesn't seem to know 62 is a US route

At that point, might as well turn it into a multi-state route 202, just like MSR 200.  :bigass:
The northern segment in DE, along with existing DE-202 can become DE-202. The rest in DE can go, since it is concurrent.
The whole thing in PA can become PA-202.
The NJ segment south of Somerville can be NJ-202. The rest is county maintained, so it could potentially get a 500-series number.
The NY segment from Suffern to Haverstraw can be NY-202. The rest is concurrent, or can be replaced by short extensions of NY-139, NY-116, and NY-100.
The CT segment from New Milford to Canton can be CT-202, and the rest can go since it is concurrent.
The MA segment from Westfield to the NH Line can be MA-202, since the rest is concurrent with MA-10.
The NH segment south of Hillsboro can be NH-202, and the segment from Barrington to Rochester, along with NH-202A, can be renumbered.
Everything in ME can go, since it is all concurrent, save the last few miles near Bangor.

You could also very easily do a NY 209 spinoff.

There were about 5-6 just in the stretch from I-86/NY 17 to Port Jervis.

Veering away from the NY situations, I found this last Saturday when heading to camp in southern Colorado. This is westbound CO 142 approaching what should be US 285 in the town of Romeo.

So many miles and so many roads

IowaTraveler

This sign heading south out of Norway, Iowa has demoted US 151 to a state highway.

In addition, US 5 has apparently been extended out of New England all the way to Moravia, Iowa.

A correct IA 5 shield is visible in the background.
