I hate to ruin the fun here, but South Lake Tahoe is definitely not in Nevada.

[pedantry]
US 89 is correct.

"South Lake Tahoe" refers specifically to the incorporated City of South Lake Tahoe, California.

Locals commonly refer to "South Shore", "North Shore", "East Shore" and "West Shore".  The casinos in unincorporated Stateline, Nevada are in the area referred to as South Shore but most definitely are not in South Lake Tahoe.

There are some green signs on I-580 and I think on I-80 with references to "North Lake Tahoe".  They are referring to the strip along CA 28 and NV 28 from Tahoe City, CA to Incline Village, NV that we call North Shore.  Nobody outside of Caltrans or NDOT would ever use the term "North Lake Tahoe" though.  The common "North Shore" is too vague for a green sign and "North Shore of Lake Tahoe" is too wordy, thus it is compromised/contracted to "North Lake Tahoe".

On I-80 just east of the OH 11/711 interchange in Youngstown:
- Hubbard (OH)
- I-79 (PA)
- New York City (NY)

Bonus points to this one for skipping a state in between (NJ).
Not to be anal but I-79 isn't a city
It is however a distance point on that sign that is located in Pennsylvania.

US-50 Sacramento has distant signs with 3 states.

Placerville, CA
South Lake Tahoe CA, NV
Ocean City, MD

Would anyone, other than a road geek, take US 50 between Sacramento and Ocean City?
I'd assume you'd take I-80 to the eastern I-76 to I-70 to I-695 to I-97 to US-50 so you'd only be on US-50 between Annapolis and Ocean City.

Can save some toll change and skip Chicagoland/NW Indiana by using I-74 out of the Quad Cities to hit I-70 in Indy and go East from there

Regardless, I doubt following an all-US 50 route would be the preferred driving routing

Google Maps suggests NE/IA 2 -> I-29 -> US 36 -> I-72 between Lincoln, NE and Champaign, IL, instead of I-80 to I-74. Guessing it's because I-80 takes you a bit north east of Lincoln.

Might there be a sign on I-75 north of Knoxville that lists the next Tennessee town, followed by Lexington, KY, and Cincinnati, OH?

Overall, I think north/south roads in KY and TN would be good candidates for that (both somewhat thin states in that direction, at least compared to east/west).

Not that I can remember, but there has long been a sign on US 25W in LaFollette, where the route splits from TN 63, that has arrows pointing left to Jellico and Cincinnati. And for years, it was spelled "Cincinatti" but I think has been corrected. (I have a picture of the misspelled sign somewhere).

Government would be tolerable if not for politicians and bureaucrats.

True Stateline is technically the city in the Nevada part of Lake Tahoe.

I'm only aware of two such instances in Iowa, both along multiplexed highways.

#1: US 275/IA 2 in Sidney:

#2: US 75/IA 9 in Rock Rapids (since replaced with a newer sign with the same destinations but postal abbreviations):

And nearby, there's this one along US 136 east of Wayland, MO:

On I-94 west of Kalamazoo, Michigan, I'm sure there's got to be a sign that reads "Benton Harbor/Michigan City/Chicago"  or something like that. Is that right?

Nope the first sign west of Kalamazoo I can recall has Lawrence, Benton Harbor and Chicago on it. It's around mile marker 55.

Are there any signs after Benton Harbor that mention any Indiana towns (Gary, Michigan City, etc.) on them? There are a few towns in Michigan past Benton Harbor - that, plus Chicago and an Indiana town, would make for a three-state sign.

I can't recall where exactly the next sign after the one at mile marker 55 (actually I looked for it, it's at the 55.4 mile marker, I knew it had Lawrence on it that's why I mentioned it). I don't recall any that have Michigan City or Gary on it. If there is a sign after Benton Harbor I can't remember if it includes an Indiana city but it most certainly includes Chicago.

Just GSVed it and didn't see any.

They lost a bet
My first trip planning trial between Sacramento and Ocean City gave three alternatives, none of which involved US-50 for any considerable distance.

A great potential candidate here, but unfortunately Buffalo, NY (135 miles from here) isn't used here. I was going to say it's not used in Ohio period, but it turns out that's not quite true. Unfortunately, the sign with Buffalo is only about 2 miles from the PA state line and past the last exit in Ohio.

I'm sure there must be at least one 3-state sign on I-10 between New Orleans and Pensacola, given that it goes through those little "tabs"  on the south end of both Mississippi and Alabama - is that right?

As an overnight guest of the El Dorado County Jail, I can confirm that gonealookin is also correct.

#44
Crossing the Poplar Street Bridge (I-55) in St Louis has signs in opposite directions each featuring three different states. It's not distance, but I think it's cool that six different states are signed.

Eastbound - IL, IN, KY
Westbound - MO, TN, OK
