There was one in Missouri a few years back. JB Hunt also had one for a while.

Not that I am aware of. The first yellow "STOP" signs appeared in New York City in 1950, and, alas, they, like other signs of the time, were short-lived. They were replaced by the 1st generation red "STOP" signs before the end of the 1950s decade.

I can think of about 20 offhand, but some of these have been last checked in on as long ago as 2006.  some serve what is now private property; none serve anything resembling a major intersection.
