I've never seen a mainline interstate use the wrong shield, though it must have happened before. I haven't sifted through the error thread in many years, maybe there's something.

You don't see many of these mixups in Louisiana. Only one I have in my collection (stolen from GSV) is this one:

Nevada is usually very good with this, especially since numbers aren't duplicated between systems.

This is the only US/State shield mixup I've ever seen: WB SR 341 approaching SR 431 & US 395 (with the opening of I-580, it's now US 395 Alt. The Street View is from September 2016, after the freeway opening, so this error might still be extant in the field)

Here's one I found in southern Utah this past week.  This is UT-261 just south of the Moki Dugway and UT-316.

I had a theory that U.S. highways in Pennsylvania were multiplexed as state highways.  My only actual evidence was that PA-519 was an offshoot of US 19.  There was a 19 shield in a keystone somewhere south of there but I can't remember exactly where.

If you enter Coggon, Iowa, from the west, you see these mixed-up guide markers taking you to Iowa 13.

I don't know if you've ever tried this, but I once took a picture of a sign error on FL 50 when they were widening it to six lanes from US 19 to the Suncoast Parkway, and the head of FDOT 7 sent it right to a construction foreman right away, who later tore it down. The post I made about it is still on this board somewhere. I'm not sure GDOT will react as quickly, but it's worth a shot.

That US 119 goof has been there for years. Surprised it hasn't been replaced yet.

On an I-196/US 31 rebuild in Michigan, they installed a couple PA 31 shields. So they didn't just get the wrong shield, they had the wrong state entirely hundreds of miles from Pennsylvania.

I won't bother posting a bunch of signs (still on my camera anyway) but as I drove from Kingston to Newburgh on US 9W earlier this week, I probably saw about an equal number of US 9W and NY 9W shields, plus at least one NY 44.
