Here is a road ( University Ave) in Dedham, MA that in a distance defaults into the ramps to and from I-95 NB after passing the overpass.

To the Left isn't any different either. Looks like a typical arterial but is the ramp to I-95 SB with a driveway into the office park just beyond the intersection.

I know in urban areas these are countless when a city street defaults to a freeway ramp, but I'm looking for examples in suburban or rural areas.
Every day is a winding road, you just got to get used to it.

Menomonee Falls (suburban Milwaukee) Falls Pkwy turns into the SB on/off ramp for 41/45 at Pilgrim Rd. (Exit 51 A-B)

I immediately thought of Boudinot Drive here in Fairfax County. It has a 0.2-mile segment connecting Alban and Fullerton Roads, but aside from that, both of its ends are highway ramps connecting to the Fairfax County Parkway at one end and the I-95 HO/T lanes at the other.

"That sounded stupid, didn't it?"

Northern end of MA 99 onto US 1 although it's a Jersey freeway.
MA 128 (solo) exit 38. Straight to take 128, turn to stay on the surface road.
I-95 MA exit 90.
I-93 NH exit 1, if it can be considered a surface road. You also have an opportunity to turn there to not get on.
Kilton Rd. in Bedford NH onto NH 101.
Somerset Parkway in Nashua NH onto the Everett Turnpike.
Ashley Blvd. in New Bedford MA onto MA 140
MA 2 exits 105 and 106, unless you have military access.

Depending on your rural vs urban definition:
1) EB University Ave east of Urbana IL defaults onto NB IL 130 which is a long trumpet ramp for I-74. To continue on EB University/US 150 you have to turn right at this signalized intersection
2) WB Church St in Champaign IL defaults onto proto I-72 leading to the 72 WB mainline and the cloverleaf with I-57

SB Oak Park Blvd defaults onto SB I-69/Purchase Pkwy at Calvert City KY near Kentucky Lake and Dam

IMO, it is important for safety reasons to incorporate freeway entrance signs in these situations.  There should be some warning as to when a normal street suddenly becomes an on-ramp.  Of course, in CA and several other (primarily western) states, the freeway entrance sign is used for all on-ramps, but I think a case can be made to at least require it in these types of circumstances, as opposed to run of the mill common interchanges.

The example OP provided is similar to how Broadway in Burlingame used to directly feed into the trumpet ramps for US 101; this configuration was changed significantly to a more conventional overpass setup about 5-6 years ago.

Extant examples in the Bay:

- Cunningham Way in San Bruno at I-280; the ramp portion is part of the former southern part of Junipero Serra Boulevard

- Oregon Expressway at US 101 in Palo Alto, though this is kinda stretching the definition of "street".  (It's however not nearly as limited-access an expressway as most of the other Santa Clara County expressway routes)

Old MA 15 that just merges into I-84 W/B right at the state line. Always a weird one to me. This could definitely function as a rural variant of one.

CA-134@I-210/CA-710 in Pasadena, California defaults to South Pasadena ave. Note CA-710 was supposed to meet with I-710 until that was cancelled.

Deptford, NJ.  Deptford Center Road, and on both ends as well.

Originally built connecting Almonesson Rd (CR 621) to NJ 41 when the Deptford Mall was built in the 1970s.  In the 1980s, it defaulted to NJ 55 when that highway was built.  More recently in the 00's, a ramp was added to the other end to connect it into NJ 42 South.

Also to note - Deptford Center Road is only a municipal roadway, but 5 lanes much of its length.

In the vein of the OP's example, this is quite common. Depending on how technical you get with urban vs. suburban, here's a few more in the Rochester area:

Lexington Ave at NY 390
Commercial St at I-490
Highland Dr at I-490

Can't immediately think of a suburban one around here, but many of the streets that run north/south through downtown and midtown Houston start and end as ramps.

Smith street begins as an off-ramp from I-10, then becomes the on-ramp for Spur 527/I-69 several miles later

Louisiana Street begins as an off-ramp from I-69/Spur 527, and travels north, becoming the on ramp to I-10 west

Milam begins as a ramp from I-45, and ends at a light where it continues onto the ramp to Spur 527/I-69

And Travis starts as an off-ramp from Spur 527/I-69 and eventually becomes the ramp to I-45 North
I love freeways and roads in any state but Texas will always be first in my heart

The end of E. Main St in Meriden, CT (old CT 66) brings you into Middlefield and the EB entrance to the CT 66 freeway. No signage that I noticed.

Liberty Avenue in Union, NJ at Vauxhall Road at its north end defaults into a ramp for I-78 Eastbound with no signs.

This might be a reason to adopt Caltrans Freeway Entrance signs everywhere.

Around Albany, Corporate Woods Boulevard at I-90 is an interesting one; it's on the city line, but the land use of the area is very suburban and the interchange was meant for I-687.

Main Street/BUS US-23 defaults to ramps to M-14 in Ann Arbor, MI. Not sure if this counts as urban or suburban, though this is in the more suburban part of the city.

Thread title aside, rural examples are OK too according to the OP, but calling this part of NY 332 a "street" is a bit of a stretch.
