If they are hitting bridges on freeways or Interstates, that is often due to negligence on the operator by operating an overheight vehicle without a permit (or using the dump truck falsely). The lowest I've seen on an Interstate or normal freeway is 13'8" on I-55 in Memphis, which is (albeit marginally) above the maximum legal height of 13'6" for trucks without a permit.

In most cases I figure they left their "PTO" switch in and accidently hit the up button for the lift or bed of the truck.

Poudre Canyon Tunnel on CO-14 in Colorado now has two different posted heights for the tunnel.  It was historically 14'5" both directions.  The CO-Trip app now has the height restriction for the tunnel as 13'11".  This is an unlined tunnel, with bare rock on both sides and above, and it clearly has different heights for each direction of traffic.

Westbound: 14'2"
Eastbound" 13'11"
