NY Yankees (more than 20, 1 was at 1996 World Series)
NY Mets (at least 3)

Boston Red Sox (at least 5, 2-3 of which were vs. Yankees)
NY Knicks (x2)
NJ Nets (at least 5, at least 2 of which were vs. Knicks)
NY Jets (no idea; several were when I was a kid - but 1 recently)
NJ Giants (Only when they were "hosting" the Jets)

I've seen games in all 30 current MLB parks plus 8 former parks.  I'm at 31 current and former minor league parks.

A lot of these were good reasons to pick up a lot of the roads I've traveled, so this isn't even all that off topic.

For the NFL, it's the Orange Bowl and Joe Robbie for Dolphins games, an LA Raiders exhibition in the LA Coliseum, and a  Bills game in Buffalo.

NHL arenas: Boston Bruins at both the Boston Garden and Fleet Center, Nassau Coliseum for the Islanders, the Meadowlands for the Devils, whatever they call the one where the Hurricanes play, the St. Pete Times Forum for a Lightning game, and Philips Arena for a Thrashers game.

AHL: Adirondack Red Wings in Glens Falls, CD Islanders in Troy, Albany River Rats, Portland Pirates.
ECHL: Florida Everblades in Estero.

NBA (though I hate to admit it): Minnesota Timberwolves.

I think that's it (for now).

I've been to a couple Detroit Tiger games at Tiger Stadium during my childhood, and once to Comerica in 2005. Also did a couple Cubs games, also during childhood (one time was SRO because Dad thought he'd be able to walk up and buy bleacher tickets). I've been to several minor league games at C.O. Brown Stadium in Battle Creek (both its past Midwest League and current Northwoods League franchises), a few K-Wings games, one Fort Wayne Komets game, and just recently attended my first Sprint Cup race at MIS!

While I haven't seen any NHL games, I have seen college hockey at NHL venues a few times, such as the Great Lakes Invitational at Joe Louis Arena.

I haven't kept count of some of the teams I've seen more than others, but I'll try my best.

Baseball:
-Rochester Red Wings (International League), many games
-Jamestown Jammers (New York-Penn League), many games
-Auburn Doubledays (New York-Penn League), 2 games (plus one rainout)
-Montreal Expos (National League, now in Washington), 1 game (my very first professional sports game attended in person, tickets were dirt cheap :D )

Hockey:
-Rochester Americans (AHL), many games (including one from luxury suite)

Lacrosse:
-Rochester Knighthawks (NLL), 1
-Rochester Rattlers (MLL, now in Toronto), 1

Soccer:
-Rochester Rhinos (USL-1), 4

...and it probably doesn't count, since it's not "professional"...but I've also attended numerous Clarkson University hockey games as well, both at home and several times at arch-rival St. Lawrence... :D

I've also seen the Durham Bulls and the Carolina Mudcats play in their respective home stadiums.

1 Tennessee Oilers game (the year they played in Memphis while transitional from Houston to Nashville (now the Tennessee Titans)
