Some other California examples that just came to mind:

I-580/Route 238 in Castro Valley was originally US 50 at Route 9 (later US 50/I-5W at Route 9).

I-405 at Route 2 was originally Route 7 at US 66

Today's I-15 junction with Route 79 south of Temecula was originally US 395 at Route 71 (though in 1934 it was proposed as Route 71 at Route 79).

I-10 at Route 1 in Santa Monica was originally US 101A at Route 26

1934 junction of Route 7 and Route 44 is now US 395 at Route 299.

Today's Kramer Junction (US 395 at Route 58) was originally 1934 Route 95 at US 466.

In downtown South Bend, Indiana:

THEN: US 31 and US 33 (concurrent) intersected US 20
NOW: SR 933 and Business 31 (concurrent) intersect Business 20

Until 1965, the intersection of Broadway and Main Street in Bel Air, Maryland was where mainline U.S. Route 1 turned onto Maryland Route 24 for a concurrency. In 1965, mainline U.S. 1 moved to the Bel Air Bypass and was replaced by U.S. 1 Business. In 1987, MD-24 moved to a divided highway to the west of downtown Bel Air, a corridor connecting the Bypass to I-95 at Abingdon. Old MD-24 became MD-924. So the intersection of Broadway and Main Street is now between U.S. 1 Business and MD-924. Same goes with Baltimore Pike at Main Street as well as Fulford Avenue at Bond Street, the other end of what is now the U.S. 1 Business/MD-924 concurrency. This is also the western terminus of MD-22. Main Street/Bond Street and Baltimore Pike/Fulford Avenue (Churchville Road east of Main/Bond) are both one-pairs here. Today, the U.S. 1/MD-24 concurrency is situated on the Bypass.
