US/State mixup shield error signs

Was scrolling through my gallery and found my US138 downgrade at its intersection with CO59 in Sedgwick.

That VA 564 shield has been there 25 years or thereabouts.  Not too far to the right is/was a set of VA 170 shields.  VA 170 was replaced by I-564/VA 337 in 1979.

US Route 197 got downgraded in Washington State:

I much prefer the rule state routes and US Routes (and Interstates, for that matter) with the same number should not exist in any given state.  This is the rule in Ohio, and several other states.  It eliminates the need for the sign in Maryland where the exit for Maryland 68 has a sign that says "this is NOT I-68"

Illinois has no problem with it. The closest* a duplicated route number gets is IL 6 on the northern edge of Peoria and US 6 just south of I-80, about 40 miles or so separated

In Virginia there's a place where a US route and a State route actually meet. US 360 and VA 360 (which is actually the old routing of US 360). Why the state didn't just call it US 360 ALT is beyond me, but here's how VDOT solves the potential confusion:

That gives me shivers.  Looks an awful lot like some ancient West Virginia secondary route (ergo, County Route) directional signage.  Except for the font, of course.  I haven't seen anything like that in the wild in many decades.

Here's a new one:  US-12 has been demoted to MA-12 or ME-12 just past the new US-127/US-223 roundabout north of Addison, MI.  Signing hasn't been completed and this might be a temporary assembly.

This is how it looks with the correct shield. There's probably a sign like that pointing to an actual state highway somewhere around there, but I didn't see one.
