There used to be one of these in Arden Hills, MN, on the ramp from Snelling Ave SB to EB I-694, preventing immediate exit onto Lexington Ave. However, reconstruction of that section of I-694 in 2012/2013 eliminated that movement from Snelling to I-694 entirely.

Luckily there's one high-resolution series of GSV images showing the sign.
I thought there was a sign like this on I-35W SB south of downtown Minneapolis.

There is still an extant one in Duluth warning traffic entering southbound I-35 from Central Ave not to cross to the MN 23/Grand Ave left exit.

There may have been at one time (not sure where it would've been...), but chances are it's probably gone now with all the construction (past or present) on I-35W.

There was a DO NOT CROSS TO LYNDALE AVE EXIT in the bad old days of the Crosstown Commons. That's probably the one.

That's where I figured it would be if it existed, but I wasn't sure enough to say. The Crosstown began reconstruction around the time I got my license, so I never got to drive the old configuration (for better or worse!)

Another Seattle one, which seems rather stating the obvious to me...

I believe the Lyndale sign is the one I was remembering.

I have on more than one occasion entered EWR airport by joining US 1-9 southbound from Haynes Avenue and then swooped across to the left exit to the airport. I don't recall seeing any signs banning this movement.

Three of the four left exit/left entrance ramps at the I-94/M-10 interchange in downtown Detroit prohibit exiting at the next right exit.  It's commonly ignored; it's not at all unusual to see a vehicle cutting across three lanes in less than a quarter mile.  The northbound to westbound ramp has no such prohibition posted, perhaps because the next right exit is 3/4 mile away.

All these locations they put up signs saying you cant cross multiple lanes in a short distance. Yet, TxDOT thought it was ok to make exiting drivers cross three frontage road lanes in a very short distance to turn right. This is just absurd.

You mean, like so (I simply turned the GSV rom the link above around): https://goo.gl/maps/eAhVhW5MqJ9iBCsEA

I don't remember where it was, but I remember one downtown Dallas frontage-road weave that was basically just a free-for-all. Cars were all over the road, no following of lane lines whatsoever.
