I've seen a couple of posts in Skyscrapercity about this, but a new ramp (not part of original plans) for Skyway Stage 3 northbound is under construction in Quezon City, around the area of Maria Clara Street in San Francisco Del Monte.  Essentially the congestion at the Quezon Avenue ramp has been so bad that this new access point has been in the works now for a while.

I've also read that SMC plans to eventually add ramps near The Marketplace mall in Mandaluyong, along the section where the viaduct runs directly above the San Juan River.  Would not only be helpful for that section of Mandaluyong, but also a portion of Santa Mesa, Manila (which has okay southbound access, but with reroutes of NLEX Connector and Skyway had not really had a clear plan for northbound access to this point).
