As for a certain route in the avatar to the left, due to the helpful improvements over the years, US 175 has only 2 turns.  Both are the on-to/off-of transitions where it overlaps Loop 7 (as well as part of TX 19 and TX 31) in Athens.  Otherwise, it has been completely straightened, and almost completely widened

Do we still count the old-fashioned "turns" where three roads intersect in a wye and the "main road" turns from one to the other?  In the old days, we treated it like a turn but said "stay on the paved road".  Nowadays, the "side road" is usually paved as well.

Dirt Roads

West Virginia is a tough one.  First off, the old roads ran alongside the rivers and creeks, but if you wanted to get somewhere faster you needed to branch off the "river road" and head up over the mountain.  Secondly, the towns in West Virginia have their own issues with terrain and the best way across town is almost never aligned with the grid (that runs parallel/perpedicular to the river or creek.  This often leaves us with "main roads" that veer off the tangent routes.

One more important point.  Because of the terrain, West Virginia doesn't have many natural crossroads.  In many states in the Midwest or the Southeast, you will have diagonal routes that come into a town from one direction, then take a 90-degree turn in another direction.  This ain't gonna happen much in West Virginia (and there's only one true example on this list).  On the other hand, the Mountain State is full of places where three roads come in and connect from different directions in a wye. 

Nobody answered whether these all are considered "turns" for this exercise.  To make my life easier, I tried to remember how these intersections use to be and hold fast to a "fork-in-the-road" rule.  If all of these were still little unsigned dirt roads, the "fork-in-the-road" rule means you still need somebody local to help you known which of these roads to take to help you get somewhere.  When I was growing up, there were zillions of such examples all over the state.  To my surprise, WVDOH has done a great job of realigning these intersections so that there aren't nearly so many "turns".

More details about my rules:  (1) Hard turning from one street to another and TOTSOs count as one "turn".  (2) Old-fashioned "fork-in-the-road" counting:  Tangent spin-offs and wyes count as one "turn" each.  (Technically, a tangent spin-off is one in the divergent direction and zero in the convergent direction).  The wye has to be obvious.  A newer road or driveway connected on a tangent should not count (and West Virginia has a bunch of these).  (3) Non-stop turning from one street to another counts, but shortened curves from one street to another (ergo, lots of corner houses removed) do not count.  Doesn't matter if the street name changes or not.  (4) Each turn in a one-way pair counts; for instance if the westbound route has 4 turns and the eastbound route has 2 turns (and all of them are different turns) all 6 of them get counted.  (5) Bypasses that turn away from an original routing count under the "fork-in-the-road" rule.  However, these do not count if the old route has been adjusted to not meet at the tangent.  (6) The "fork-in-the-road" rule does not apply for four-lane roads unless the tangent road is also a four-lane road.  For simplicity, it doesn't matter which road was four-laned first.  (7) Routes exiting the Interstate after a multiplex then turning onto a perpendicular road are only counted as one "turn".  It is actually two in the exit ramp direction and one in the on-ramp direction.

One exception:  Rig Road off of US-220 north of Petersburg follows along the west bank of the South Branch of the Potomac River.   The US-220 bridge was replaced after the 1985 floods, and WVDOH has improved the intersection so that it should be barely disqualified.  But it is still so obvious that you are turning off of the "river road" to cross the bridge that I felt it deserved an exception.

Two-fer:  City View Drive connects to the switchback on US-250 in East Moundsville in such a way that it is virtually tangent with both the [eastbound] and [westbound] lanes.  I would have normally disqualified it as a "newer road", but other aspects of the "fork-in-the-road" rule put in squarely on the list.  Counted as only one "turn".

How about staying on the same "straight road" but needing a TOTSO to do so?  US-52 and US-460 are multiplexed from Blueprince to South Princeton along the Old Cumberland Road path that runs along the north side just up from of the base of East River Mountain.  But Corridor Q veers north topwards Princeton and takes US-460 with it.  US-52 exits Corridor Q and stays on Cumberland Road to get to I-77 at the East River Mountain Tunnel north portal.  When this used to be a stoplight, I would have counted it as a "turn" for both US-460 and US-52 under Rule #6 above.  With the interchange, only US-52 gets a "turn" for the TOTSO.  This actually happens in a lot of other places, but this is the only location in the Mountain State involving US Routes.

*The zig-zag in Fairmont that takes US-19 from Locust Avenue to Jackson Street doesn't count.  But it sure looks like it makes two or three additional "turns".
**The straight movement for US-33/US-119 through the roundabout in downtown Glenville doesn't count.  It's the only one on US Routes remaining in West Virginia (and I don't remember it being there many years ago).

As much as I've yowed about the wyes, I only found two that I would still count as "turns":  US-19 intersection with Sammy Run Road nears Walkersville, and the US-219 intersection with WV-24 in Silver Lake.  The first one is a simple wye, the second one is a complex wye.  Interestingly, WVDOH has converted a number of simple wyes into complex wyes by straightening out the "main road", which eliminates the "turn" from the count.

Keep in mind that the earth is spherical.  Even routes that appear to go straight have unseen turns. :)

Also vertical curves.  Even the few straight stretches in West Virginia are probably on vertical curves.

Perhaps the goal should be to find the route in a given state that has the MOST turns rather than an attempt to catalog them all.

NY 21 has a bunch... let's see:

That's even more than I thought. It's interesting that four different routes terminate at NY 21 at an intersection where staying on NY 21 requires a turn (NY 64, NY 53, NY 371, and NY 63), and two more routes terminate while concurrent with NY 21 (NY 332 and NY 15).

So, you could definitely say that NY 21 is NY's most persistent state route given the way it interacts with those 6 different routes at locations where it could logically terminate, but it always ends up continuing while the other routes are terminated.

There are many places where there used to be a turn, but the road was redesigned so that there is a tight curve with right-of-way over the other roads, so I don't count those. And I only count a traffic circle if the road wouldn't otherwise go straight through.
