Tuscaloosa. Currently it would be US82 but when I was born McFarland Boulevard didn't exist so I think AL215 was on University Boulevard at that point.

The old Deaconess Hospital in Cleveland was pretty much at the corner of Pearl & Broadview Roads -- US-42 / SR-3 and Old SR-176 (before it was relocated to the finally-completed Jennings Freeway).
