How about NJ 4 and CR 4 in Nassau and Suffolk counties in New York?  There's photos of shields for them on my site, except Nassau CR 4, because Nassau County doesn't post its county route numbers.

How about route numbers that used to be 4 and are four years old (i.e. were changed four years ago).
Mileage-based exit numbering implies the existence of mileage-cringe exit numbering.
