Noticed that myself several weeks ago when they put a corrected US-422 shield up in what seemed to be a bit of a slapdash manner.  I couldn't figure out what was wrong with the numbers -- they seemed to me to be barely contained by the left side of the shield.

This is on U.S. 400 near Strauss, which is about nine miles east of Parsons in Labette County. And it's not the first time Ohio shields have appeared in Labette County. When the bypass around Parsons first opened, the contractor posted the entire thing with Ohio 400 shields. My guess is that either KDOT or the county kept the signs when they corrected the mistakes and then used one of them by mistake to replace a missing U.S. shield.

They're correct in this GSV link, but the US 11 shields have been replaced with incorrect NY 11 shields on this assembly in Whitney Point.

This is a US/state route mix-up error shield from New York that I recently purchased. None of the holes have been punched through the sheeting, although it clearly saw a little bit of abuse while in storage. The upper left edge was also a bit screwed up in fabrication, since the black doesn't completely fill in that corner.

Either I took a wrong turn at Albuquerque, or Oregon DOT botched a sign. Not sure which  :-D

OR 201 just west of the Idaho line, intersection with Succor Creek Cutoff, Malheur County.

Due to the cold & being pressed for time; I was not able to get a photo of such last night when I saw it, but an erroneous US 261 shield has recently been erected along DE 92 (Naamans Rd.) westbound shortly before the intersection with DE 261 (Foulk Rd.).  It replaced an older JCT. 261 assembly that had the correct DE 261 shield (shown in latest July 2018 GSV) that was likely either vandalized or destroyed in an accident.

Technically this assembly in Golden, CO, is correct...but US 6 doesn't meet US 93 until Ely, Nevada. Should be CO-93.

I drove past a sign for "NY 20" last week.  As I looked at the signs, I thought something was off, but it took me a second to realize the shield was for NY 20 instead of US 20.  I think what was throwing me off was that the numbers in both shields are thinner than normal too.

I don't have a picture, but a month or so ago, I saw that the shields for US 11 in Cicero at this intersection were replaced with NY 11 shields.

Two things wrong with this sign in Colbert, OK. One, it should be US 69. Two, they used the old circle state highway sign instead of the meat cleaver that went into effect more than a decade ago.

They got it right on the LGS in the background.
