Turn/Curve + Advisory Speed Sign with Supplemental Plaque

Are there any places with warning signs with turn/curve arrows and advisory speeds on the top and supplementary plaques on the bottom? One place I know is on San Jose Ave eastbound near Rousseau St in San Francisco, though the top sign has a rare 135-degree arrow.
