According to http://www.nycroads.com/roads/grand-concourse, the Grand Concourse had exit numbers in the 1980s and 90s. There are still remnants of it today, including two numbered signs (NB and SB for Fordham Rd.) and several patched exit gore signs. My question is whether there's any source that lists the old exit numbers. I've been unable to turn anything up.

Send a FOIL request to the NYCDOT. If they have anything, they have to give it to you. Although I'll warn you that NYCDOT really hates dealing with the public.
Considering my position in the traffic industry, which involves NYCDOT on several efforts, I'd rather not bother them, even just as a public citizen. (You never know who'll recognize a name.) I guess there really isn't any outside documentation of it.
