I think I have a photo of a quadruple banner in Norton, Va. Either "TO OLD ALT 58" or "TO BUSINESS ALT 58."

that's only 3 banners. 4 if you count the shield. 5 if you add an arrow.
this gantry has four banners.

Not as many, but separated more.
Anyone know of a "To Temp Truck Alt (direction) (route)"?

I would like to just see multiples of "truck", "alt", "bypass", "business", etc. they don't seem mutually exclusive. the only combination I can remember offhand is "business alt" -- I think there's such a US-98 in Florida.

Nope.
There's a Truck Business US 17 in Elizabeth City NC: http://www.vahighways.com/ncannex/route-log/us17b.html

I remember seeing these last fall. Didn't get a pic because it was dark.

It may have a direction. I need to wade through my photos.
I also think it may be a cutout. Bonus points!

I've got a triple in town, but no quads.

Why does a Business route have a Bypass? That's what the Bypass route is for. And that's not what the Business route is for.

I think they're trying to say that the business leads to the bypass. Of course when you get to the bypass it's no longer the bypass, unless you then turn the wrong way. So it should read
TO TO
WEST EAST
BYPASS BYPASS
EAST WEST
BUSINESS BUSINESS
PS: the Florida example is out-of-order; it should read
TEMP
TRUCK [ROUTE]
TO
SOUTH
39
since the truck route itself is to the right. Or just
SOUTH
TEMP
TRUCK
39
but FDOT tends to be overly wordy wrt truck routes. Another sign on the truck route itself (again out of order):
