Various Utah cities do not count, the block number is the Street Name.

The US 19 freeway in Pinellas County, Florida has block numbers on guide signs routinely posted for the accompanying frontage roads.

IIRC, NB I-55 in St. Louis has block numbers on advance signage to differentiate the two exits for S. Broadway. Not sure if that counts, though, since I think the BGS's themselves just say Broadway.

I swear it's done in Michigan somewhere. But I'm probably mistaken. Either thinking of Chicago doing that or other MI oddities, like having clearance signs on a bunch of the overpasses.
