On I-24 northwest of Nashville there are mile markers way out in the center of a wide grassy median, almost difficult to clearly read at 70 mph, with signs facing both directions of traffic mounted to a single post. They are the new shield-displaying ones, every .2 miles.
I've seen this on roads with Jersey-barrier division (like the Mass Pike) but never in a big wide field of grass. Is this unique, or does it exist elsewhere?
Given the small lettering so far off the road, it felt a little like getting a mediocre result on the cheap.
These are typically only used on urban sections. Urban sections typically have narrow median with walls. Sounds like these are in a rural area that is urbanizing, but the freeway is still rural style.
Sounds like they failed to adapt to a unique situation properly.

Here is one that was on US 31 near Kokomo on what is now SR 931.
Alabama has mileposts on a single post facing both direction on all state maintained 2 lane roads.
I-64 in Virginia between Newport News and I-295 east of Richmond has double-sided mileposts along the eastbound lanes; the reverse side is for contraflow operations. Each "reverse" milepost is appropriately marked "REV I-64".

Parts of I-49 (and US 71) in the Kansas City area have mileposts on the Jersey Barrier in the center of the roadway
Pretty common in parts of Ohio with enhanced location markers. Even when carriageways are so far apart that a single post can't be used, the marker is on the left. Indiana is the same way.

Won't be so far removed from travel lanes for long. I-271 is being widened between SR 8 and US 422. Extra lane in each direction for the entire stretch.
Indeed, but the tenth-mile markers have been way in the middle like that since they went up long ago when they could have put them nearer traffic--probably saved on knocked-down markers!
The third lane is sorely needed...the couple miles between 8 and 480 can get pretty bad.

I know. Also quite bad where the express lanes merge in SB. Express lane will become the left lane and (eventually) the SR 8 exit.
I was through there 2 weeks ago and was quite amazed that they were actually widening it. Area around I-77 is also getting bridge replacements and possibly a full-depth reconstruction.
That work zone is a mess. Down to one lane each direction on the SB side.
