Title: Closed Entrances

We have a thread for closed exits, but not one for closed freeway/expressway entrances, so I figured I'd start one.

I'll start:

-NY 33 WB just east of NY 198 has a closed entrance from Fillmore Avenue, in the early 2000s
-New York's I-190 once had an entrance ramp from Porter Avenue to the SB side that was removed when the Peace Bridge got direct ramps
-I-70 WB/71 SB in Columbus, Ohio had an entrance ramp from NB/EB Mound Street immediately east of the now-removed bridge

I-84: EB from Woodford Ave in Plainville (closed when CT 72 expressway west of I-84 opened in 1980).  Also CT 72 WB from Crooked St. when the 84/72/372 interchange was rebuilt circa 2000. 

I-84 WB entrance ramp from Trumbull St. in Hartford.  Part of late '80's I-84/I-91 interchange project.  Plus there's one about 1/4 mi away across High St. along N. Chapel St. 

A couple of entrances to I-91 North along Columbus Blvd. in Hartford were consolidated into a single tunneling entrance to I-91 North and I-84 West as part of the CT Convention Center project.

Couple near me:

WA-512: WB entrance from 31 Ave was deleted when another interchange a few hundred yards away made it redundant.

I-5 to WA-512: EB entrance loop from I-5 was removed and replaced with a signal (remnants still exist).

I-5 to WA-18: WB entrance loop from I-5 was removed and replaced with a flyover (remnants still exist).

The Spokane St Viaduct in Seattle used to have an entrance from 4 Ave, but it was deleted a few years ago.

I mentioned the ramps from Westshore Boulevard in Tampa to I-275 were closed in another thread, but I will mention it here as well.

A few more from CT:
* Bina Ave. to I-91 NB, Windsor; closed with I-91 reconstruction c. 1992. This "interchange" had only the entrance ramp.
* Main St NB to I-91 SB, Windsor; closed earlier than I-91 reconstruction. Originally a 5-ramp interchange.
* Broad St NB to I-84 WB and EB, Hartford; closed c. 1990. You can see an extra lane begin on I-84 EB where the Broad St. entrance merged in from the left.
* Spring St/Bantle Rd (SR 519) to CT 2 EB, Glastonbury; closed c. 1989.

Chicagoland:

Special Case:

Beavercreek, OH:
Research Rd. to US 35 Westbound.  Much of the alignment had a bike path access placed on it:

For whatever reason, an entrance to southbound VA 76 from Douglasdale Road in western Richmond, VA was closed before 2007. The ramp was used for storing construction equipment for a while, but has remained closed even after the equipment left. I imagine the RMA (now the RMTA) considered this entrance redundant with an entrance on McCloy Street about half a mile away.

Does this count? It's the old Mass Pike ending at MA 102 in West Stockbridge yards from the NY border.

The former entrance from Bayshore Boulevard to US 101 north in San Francisco (at the Silver Avenue exit), blocked off about a decade ago as there is a second entrance a half mile north at the Alemany Boulevard exit:

How about the I-295/I-64 interchange in Short Pump, VA (near Richmond)?

Originally, the T-interchange was a half-cloverleaf, complete with short ghost ramp stubs continuing on.  But one of the loops was closed when the EB->NB movement was replaced with a flyover.  A path through the brush, along with maybe some pavement, remain today.

We have a kinda closed entrance on Memorial Parkway here in Huntsville:

However, the entrance will be getting use in the next few years as they'll be building some more overpasses along The Parkway.
There was also this un-used stretch of pavement just to the south that would've allowed traffic on Blanda Drive to access Memorial Parkway, but it was torn out recently:

On I-94 in Michigan, near Battle Creek around the 90 MM, there is a closed entrance for the former Fort Custer army base.  This was used to allow military vehicles direct access to I-94.

OP took the interesting ones, but in Utica there was a ramp to John St from Genesee St. that was recently closed.

The on-ramp from Quarantine Road to I-695 East (outer loop) in Baltimore City was closed several years ago and returned to natural condition. It was apparently deemed redundant as a separate on-ramp to the Outer Loop from Fort Armistead Road was built many years ago and provided an alternate merge point for the many large trucks using the roads in this area.

There's one just east of Eureka, MO in I-44 from old US-66 in Times Beach. The onramp from old US-66 onto I-44 WB was part of exit 265 (now for Williams Rd) but was abandoned when Times Beach was contaminated and evacuated in the mid-80's.
