Title: "Remorseful" FYA installations

This is County OO and County AA in Grand Chute, WI.  FYA arrows were installed, but were later converted to protected left turn signals as the sign above indicates.  Any other cases where the FYA was experimented and switched to a protected left turn signal?

There are several FYA installations in Bellevue and Redmond, WA that used to operate with the flashing yellow phase during the crossing phase, but were then were switched to FYA only during the non-crossing phase, due to some collisions involving pedestrians. I've never seen one that was straight up disabled, however.
