Dallas North Tollway at I-635 appears to have five. It has the mainlanes and C/D lanes as well as managed lanes. In Street View, it looks like the managed lanes are on one structure. The westbound C/D lanes are definitely on a separate structure, because the beams are at a different elevation. The eastbound C/D lanes appear to have their own structure, as shown in the linked view, but I'm not certain.

I-69 at US-127 east of Dewitt -- this one cheats a little since the southbound US-127 C/D lane ends before the ramp from eastbound I-69 merges in. That ramp, by the way, used to be the mainline before the freeways were extended to the north and east. Also note how all four loop ramps appear to have different radii.

The defining characteristic for this thread is four separate overpasses. The OP said "typically" that will happen with C/D roads as an example, but frontage roads (or express/local lanes or anything else that might cause this) definitely qualify.
MD-32 and US-1 in Jessup or Savage (the interchange might actually straddle the two municipalities).

Yes there are. I drive through there every day. The C/D lanes and through lanes have almost no gap between but are separate structures.
