For some reason I remembered a sign I saw a long time ago in the Goofs and Oddities section of AZ roads. It was (and still is (https://www.google.es/maps/@32.9440014,-112.7309137,3a,18.9y,321.72h,87.82t/data=!3m4!1e1!3m2!1sv34lRlDSPoJLWjPAsjlIcA!2e0) as of May 2015) at Westbound Bus I-8 in Gila Bend, with San Diego being straight ahead and Mexico to the... right? We are going West, so shouldn't Mexico be to the left? Actually it happens due to AZ 85 looping to cross under Bus I-8 and a railway.
Of course, examples can be from around the world, not just the USA. I myself encountered this one (https://www.google.es/maps/@42.7611436,-1.6326618,3a,59.6y,109.62h,80.56t/data=!3m4!1e1!3m2!1sfUpaB846igZyjVO99YaR0Q!2e0) just South of Pamplona, Spain, in early September. This is caused by the A-21 on-ramp joining the ramp from southbound AP-15 and then overpassing the latter, resulting in the entrances to SB AP-15 (going to the right, no NB entrance) and A-21 (straight ahead) being switched.
This GSV (https://www.google.com/maps/@43.7513304,-88.4620919,3a,75y,359.64h,79.79t/data=!3m6!1e1!3m4!1sri0PsF8NhP91p2QsRDCdpA!2e0!7i13312!8i6656) from Fond du Lac, WI, points northbound, but the reassurance sign reads "South County VVV". This is the only VVV sign on the entire street, too - maybe there was some confusing decommissioned alignment that used northbound Hickory Street as its southbound alignment and they forgot to take the sign down?
