I apologise if there's already a thread on this but after doing a search I can't find anything so here goes.
Is this setup rare or not? On all my trips to the US I have never seen such an arrangement before. I've seen individual signs put together in a bunch but never a single sign with multiple shields.

It's common VDOT practice in the cities of Richmond (and surrounding area), Newport News, and, to a lesser extent, Williamsburg.

On Mercury Blvd westbound (US-258 SB) at Jefferson Ave (VA-143) in Newport News.
It's a brand new sign, so I'm not sure why they still left US-258 on a seperate sign as if it were an afterthought.
And do I spy a US shield for OH-37 or the other way around in that pic? ;-)

Like pretty much everything else, it depends on the state. The "backing board" approach tends to be rare for permanent signs (other Virginia examples include confirmatory assemblies on I-85 northbound between Petersburg and the NC state line), but in Missouri it is now used extensively for construction detour signing.
Ohio is a nice state to have recognized US 37.

Wisconsin does this a lot, though they often leave the arrows and banners out of the grouping:

Franklin, TN has put at least one of these unisigns downtown where Bus. US 31, Bus. US 431 and TN 96 meet. (with a TO I-65 sign added in for good measure.) And, yes, getting a picture is on my list of things to do. There are also a few at the Exit 67, the McEwen Drive SPUI on I-65 in Franklin with the direction, shield and white on blue arrow on the same sign.
I'm pretty sure that it is the city of Franklin doing it and not TDOT as I haven't seen any signs like this anywhere else in Tennessee that I can recall.
Washington does that a lot for reassurance shields like this:

I guess you could make the argument that Nebraska uses them quite frequently at junctions, but I'd argue that's a whole different animal
That's a pretty new sign there in Delaware (OH, the first picture on this thread). Not a big fan.

Lately, though, the trend has been to include directional banners and arrows. Much of the signs along US 2 looked to have been replaced within the past year. (Yes, US 2 and the Circle Tour are on a single panel.)

That's not a bad idea. It's a lot clearer than having multiple shields. An example of this I'm familiar with is US 1-9 in New Jersey.

They'll have to bring them back when the 2009 MUTCD goes into effect. It specifically disallows anything other than green as a background for this type of sign.

Even if it's just a reassurance shield? I'm not sure why that first 395/20 one would be illegal on a one piece. It's constructed exactly like 2 separate shields posted next to each other except it's just one

Gah! Must be Delaware OH....
For sure...My sister lives just outside of Delaware, OH and if you are not passing thru the town on US 23, then you need a sharp eye and some luck to keep on the same route through town.
Not only that, but Delaware is the city full of NIMBYs who don't want to see US 23 improved/upgraded into a 4-lane expressway, nor do they want to see an I-73 shield anywhere near their town. Yet they love stop & go traffic congestion from Delaware south to I-270 in Columbus.
BTW, for those keeping score at home, the obstructed shield on the BWS is US 42.

VDOT's Richmond District seems to have really taken to the "unisigns". I've only rarely seen them elsewhere in the state.

According to the FHWA's Powerpoint notes on the MUTCD, when route shields and their associated auxiliary messages are combined on one sign, the background color *should* be green (which equates to a recommendation, not a requirement). However, the proposed text specifically disallows just mounting route shield auxiliary signs to a guide sign background.
