You mean flashing reds? In Harford County, Maryland we've got MD-136 at MD-440 ("Dublin"), MD-23 at MD-146/Madonna Road ("Madonna"), MD-136 at MD-624, where MD-136 actually has a flashing yellow, and then there's a three-way, MD-155 at MD-161 outside of Havre de Grace.
Lots of 4-way stops/flashing reds on US 45 south of I-70 in Illinois. Heck, lots of them in Southern Illinois in general.

Not just Southern, there are some on IL 1 between Chicagoland and Danville, IL (more Central, unless SoIL is everything South of I-80)

Quadrupole faced single signals that face all four directions, not 4-Way clusters.
There's a pair in Baltimore at Charles St and Montgomery St. just south of downtown, and that's just one I pass by daily. They're still scattered around the city though they're being replaced slowly.

Yes, single housing, not four separate housings. I think the conversation diverted to 4-way stops very quickly because of confusion over 4-way what.
There are still some around in places east of Cleveland but far, far fewer than there were even 10 years ago as a number of projects have replaced everything on some corridors, including signals.
I just saw a couple old 4-way signals fairly recently, although I can't remember exactly where in running around they were! Sigh. Didn't have time to snap a pic.
There's still a few on SH-19 in Lindsay, OK.
None in Huntsville or Madison County, AL for that matter, but there are several in the surrounding towns. Fort Payne, AL and Athens, AL have quite a few, along with Gadsden, AL. Maine is completely devoid of full R-Y-G fixed 4-way signals as far as I know. There are a few beacons left that I've seen listed below, but those are few and far between.

Come to New York if you want 4-ways. Troy in particular still has them all over the place. All are on span wire and, at most intersections, there is only one face per direction. Multiple 4-ways are within walking distance of my apartment. A large amount of the 4-ways are on ancient controllers without an all-red interval.

There are at least four 4-way, single-segment flashing red blinkers in my neck of the woods. All four are at secondary road intersections. Otherwise, I got nothin'.
Nothing in my area. Fayetteville was closest, but they got rid of theirs 3-4 years ago. Same with Arkadelphia.
Smackover, AR has one, but it's the only one I have knowledge of in Arkansas.

That's a 4-way
cluster. Not quite a true 4-way, but many towns seem to do it to directly replace 4-ways with newer signals (without having to install any new mast arms or span wires) or to try and maintain the old look of the 4-ways while installing newer signals.
Having just returned from Texas and Louisiana, I didn't see any 4_ways, though I've previously spotted this in Jefferson, Texas

If I didn't know better, I would have thought that picture was taken in Michigan (4 way clusters that look like that are very common here).

As far as I know, there are none remaining in Knox County, Tennessee. Perhaps there are some lurking in the most rural corners of the county, but none by me.
The only flashing 4-way I remember is outside Macon, GA at the intersection of Lower Thomaston and Old Saxony Roads. It's been decades since I've last been there, so it may very likely be gone.
There are a couple of old true four-way signals that are still in Michigan. The first is a Crouse-hinds type M in Croswell. The second is a Sargent Sowell in East Tawas, but it has been in permanent flash mode since the 80s. I'm new to this forum and I don't know how to add pictures to posts, so I'll just put the links of a YouTube video I made for each of the signals.

Michigan has a lot in rural areas mainly, not so much in urban areas but they do exist. In Saginaw County there are several 4 way stops or all way stops as they are usually called. There are some 3 way stops as well.
I know that for example the corner of Swan Creek Road and M-52 in Saginaw County has a nightly four way stop, it's a regular traffic light during the day.

clusters

"Sargent Sowell" is a tough name for a signal brand
Sargent Sowell was a police equipment supplier in Grand Prarie Texas, not really a signal manufacturer.
They contracted out their signal manufacturing to Traffic Signals Inc of Louisiana.
TSI sold to TEECO, another police supplier, that continued to make this signal style until the mid 2000s. You could buy them new from TEECO's site for about $2,000 a pop.

Last night I clinched M-52 and about 2.5 miles south of the northern terminus is a nightly four way stop. I had always wondered why it was a red flasher for a state highway crossing a more local road but then I thought it's probably like that to slow M-52 traffic down to avoid accidents. M-52 is 55 mph in that stretch, Swan Creek Road doesn't have a posted speed limit but most people do about 55 mph on that road as well. It's the only four way stop I came across on M-52. I was in Jackson and wanted to keep driving south and sooner or later was going to have to turn around and head back north so I decided to do a clinch of M-52, I also clinched M-99 last night but I've clinched that highway before.
