If an Interstate and US route parallel each other, there will probably be places where they cross but there is no interchange.
But what if they cross only once? Most times, there is an interchange (such as I-64 and US 45 in Middle-of-Nowhere, Ill., but sometimes there isn't.
Kentucky examples:
I-64 passes under US 31 (Clark Bridge) in Louisville.
I-64 crosses US 421 near Midway. However, access is available via KY 341 and US 62. Once upon a time, one of the eastbound guide signs had a "To US 421" notation on it, but it was later changed.
US 25 crosses I-64 near Lexington with no interchange.
I-65 crosses US 150 (Broadway) in Louisville with no exit.
I-65 also crosses US 60 (Main Street and Market Street) without an interchange.
I-40 completely ignores US 301 in Benson, NC
In East Hartford, CT, US-5 leaves the Wilbur Cross highway to become Main Street. IIRC you can get on to I-84 west from US-5, but that's the only direct intersection of the two.
I-71 and US 30
I-71 and US 35
I-95 and US 360
I-95 and US 60
I-77 and US 64
Iowa:
I-380 and US 6. I'm pretty sure that's the only one.
Illinois:
I-88 and US 52
I-280 and US 67, although the nearby exit says "TO US-67"
I-90 and US 7
I-87 has a TON: US 202, US 6, US 44, US 209, and US 20. Most of those cross within a mile of an exit, so access isn't actually difficult, but US 44 is more than four miles from an interchange. All of these are on the Thruway.
I-90/US 11 also counts. Yet again, on the Thruway, but close to exit 36 and easily accessible via I-81. I think I-90/US 62 as well (yet again, Thruway).

The only ramp is US 5 North to I-84/US Route 6 West, between the interstate's Exits 57 (CT Route 15 South) and 56 (Governor Street).

I-294 and US 45 comes darn close in IL. There is an Entrance to I-294 South from US 45 just south of the Mile Long Bridge/North of the 82nd/83rd St Toll Plazas. No Direct Entrances to I-294 North, and no Direct Exits from I-294 to US 45 (plenty of indirect ones).
Of course, that US 45 South to I-294 South ramp also functions as the Missing I-55 South to I-294 South Ramp (US 45/LaGrange Rd is Free Flowing between the two Interstates, across the River/Canal/Rail Lines/Mile Long Bridge Stuff)
US 45 and I-294 cross twice, once near O'Hare and also near the aforementioned Mile Long Bridge
That said, the 2 do cross twice, not once, and there is at least a partial movement available
Over to the Buckeye State now, how about I-76/OH Turnpike and US 62?
Not only do US 19/41 pass under I-20 in Atlanta with no interchange*, but so does US 29 just 1000 feet to the west. None of the three routes cross I-20 again.
* Eastbound exit 56A is signed as US 19/29, but should properly have a "TO", as it spits you out three blocks away from those routes.
I476 and 202, crosses just once near philly, no connection.
I’d bet this is more common on toll roads.
I-44 ignores US 177 in Chandler, OK, opting instead for an interchange with OK 66.
I-335 misses US 75 in southern Topeka. The movement between those two highways involves a connection on I-470 and surface streets.
Before US 40 was decommissioned west of Park City UT, it never had an interchange with I-15 in Salt Lake City.

I-80/I-90 and US-23

I'd consider the ramps to US 5 in the I-90/I-91 interchange as an intersection.
If I recall correctly, US 319 crosses over Interstate 75 in Tifton, Georgia, but there is no direct interchange between the two routes. See here. I've always found this to be incredibly interesting. US 319 crosses I-75 once (right here), and it never sees it again.
This is the only example that comes to my mind right now.
I-95 and US 27 in Miami, though both terminate at US 1 only a few miles apart.
I-44 at US-177. Although there is a very close exit (https://www.google.com/maps/@35.6849995,-97.0295525,15z), so does this count?

I-70 crosses over US 11 in Hagerstown, MD, just east of the former highway's interchange with I-81.
I-91 and US 1. US 1 crosses under right at I-91's southern terminus as it merges into I-95.
US 78 crosses over I-459 without an interchange.
Similar to the Tifton, GA example, US 319 used to cross over I-10 without an interchange in Tallahasse as the exit went to FL 61. When I-10 was widened a decade ago, an interchange was added from I-10 westbound to US 319.

They are both very significant highways through the region, and as has been noted, they cross eachother in a relatively rural area, so that really is surprising. What's very fascinating is that it looks like there are two different interchanges on I-75, near where it and US 30 cross, but neither of the interchanges are directly for US 30 itself. One is at Napoleon Road, and the other is at OH 696 (Lincoln Highway). Also, US 30 has an interchange with the latter road right there. So while just barely, it is indeed true that US 30 and I-75 don't technically directly intersect.

As noted, there was an existing interchange with Old US 30 (Lincoln Highway, now Ohio 696).
I suspect that the extreme angle where the US 30 freeway crosses I-75 made a direct connection difficult. Plus the railroad parallel to I-75 would have made ramp bridges interesting, to say the least.
I believe I-64 and US 58 qualifies for this.

Even though it's not signed as such (and is erroneously signed as US 33 East instead), the ramp at Exit 74C (NB) dumps traffic directly onto US 360 WB.

US 92 at I-75. Though its very close to the I-4 and I-75 intersection which is why there is no connection here, it has easy access between the two using the one digit interstate.
US 202/206 at I-78 in Pluckemin, NJ cross but have no ramps between. Again close to I-287 & I-78's exchange, with easy connections via I-287 except on NB US 202/206 as a jersey U turn is required through a jug handle north of the US 202/206 and I-287 interchange (no direct ramps to the SB freeway there) its still not that far out of the way.
