It's a little green diamond with an exclamation mark and the word "watched" in green text, which appears beneath your avatar. Only the watched user (and I assume mods/admin) can see it. Ditto for "moderated", except that's orange text instead of green, and that requires physical moderator approval before your posts display.
Not that I want to publicize all that knowledge, but it is what it is, and it does (I think) answer the OP's question :-P

I've received a few warnings before, the most significant being a thread I created that was a proposal to temporarily ban someone. I've never received the "!Watched" warning, though.
It seems to be a points system, in which points are bad, but are set to automatically decrease at a rate of one per day.
I switched from "moderated" to "watched" at around 30 points, and then dropped off the watch list altogether at 10 points. So I assume "watched" only appears for accruals of more than 10 points.
Credits (negative points for good or improved behavior) can also be issued. And with that, I think I've said enough.

It's probably possible, for minor offences, for the mods to issue a warning without assigning points.
In my case, the warning message told me how many points I'd been assigned.

This is how it works (an explanation I received several years ago when I was adjudged to have been picking on NE2):

A level of 24% simply puts you on the moderator watch list. (Beyond a certain level posts require moderator approval before becoming publicly viewable, and at 100% the user is muted.) Simply put, as long as you don't accrue any more points you don't have anything to worry about as the warn level will slowly tick back down to 0%.
