Noticed while recently driving I-205 in Vancouver, WA that on northbound 205, there is one exit ramp at the Mill Plain Blvd exit that serves three lettered exits. 28A is Mill Plain eastbound, 28B is Mill Plain westbound, and 28C is 112th Ave. All three are served by one ramp off of 205.
This seems unique, but I'm sure there's other places similar?

The NJ one has it because SB has two ramps serving the same place as the NB one along with there being a 2A at the u turn ramp back to I-295 just before it creates the awkward situation.
The EB I-4 ramp to Michigan Street in Orlando use to be 81B-C before the I-4 Ultimate. That too had a WB Exit 81A with two WB Michigan ramps to create the same exact situation.
Per the MUTCD, states decide their own policy with regard to numbering exits on ramp splits and CD roads. My own state of Georgia doesn't, but obviously many other states do.

Once complete, there will be 51H (I-290 - Aurora), 51I (Congress Pkwy/Ida B. Wells Dr), and
51J (Taylor St/Roosevelt Rd) heading southbound (eastbound) on I-90/94.

The new draft of the MUTCD lays out clear examples on how CD ramps are to be numbered and signed. This is needed as they currently differ from state to state (or even interchange to interchange).
US 12 NB in Wisconsin, EXITS 330 A-B is one ramp for the entire interchange.

I wish I could remember where it was, somewhere in Wisconsin I think. The direction we were heading had only one exit ramp, and you turned left or right at the end of the ramp. The other way had 2 exit ramps, one a normal diamond type and the other a cloverleaf ramp. In both directions, the exits were numbered XX A and B.
