I've driven by this sign this sign (https://www.google.com/maps/@39.9161565,-84.1882293,3a,15.3y,12.6h,93.09t/data=!3m6!1e1!3m4!1sW7gINghHHYqxIHL2CcSg!2e0!7i16384!8i8192) millions (well, maybe hundreds) of times. It always makes me wonder if there are other mileage signs listing 3 or more cities all starting with the same letter.
Is anyone aware of any others?
If I had to guess, I would suspect somewhere in CA with all the San and Santa ciities.
There's one on eastbound US-211 near Sperryville, Virginia. This one is there because of the brief concurrency with US-522. Otherwise, they wouldn't have the third "W" on there because the sign would have Washington and Warrenton, but there's no other notable "W" along US-211 (Waterloo is too small) since the road was truncated some 40 years ago or so. It used to run all the way to the other Washington (DC).

I'd be willing to bet that there's probably a sign somewhere along or near US 23 with Paintsville, Prestonsburg, and Pikeville on it. It might not be a mileage sign, but could possibly be directional arrows.
Looking at the map of southwest Minnesota, it seems somewhat plausible that MN 60, east of St. James, would have a sign for Madelia, Mankato, and Minneapolis (the latter accessed via US 169, which - coming through Mankato on the MN 60 expressway - is actually the same roadway). Is that actually the case? I'd like someone from Minnesota to chime in on that.

Only Madelia and Mankato are signed. With a couple exceptions such as Mankato as a destination for MN 109 coming off I-90, MN is not big on signing destinations a route doesn't actually go to or near.
I had checked another possibility on I-94: St. Joseph, St. Cloud, St. Paul, but it looks like the mileage sign right before St. Joseph is just a generic "Exits to St. Cloud" sign. Another one, Sauk Centre, St. Cloud, St. Paul, unfortunately it was Minneapolis's turn to be on that sign.

Not a mileage sign, but similar: This is a destination sign with 3 cities starting with the same letter. https://goo.gl/maps/zyTT1DPqNLA5fDRt5 Considering that GA 10 and US 78 do not reach Winder, this is likely a carbon copy of an older sign from when US 29 and/or GA 8 were routed on that road.

Is there a sign saying "Charleston, Cairo, Chicago" or "Charleston, Champaign, Chicago" at the start of I-57 near I-55 in Sikeston, MO?
Or perhaps "Carbondale, Champaign, Chicago" further up I-57, across the river in southern Illinois?
There's one or two on US-51 at the WI-70 intersection north of Woodruff that say Manitowish Waters, Manitowish, and Mercer.

And what's more, going in the other direction from there, you could have one with Minocqua, Merrill, and Madison.
It's not quite what the OP wants, but this sign on MN 61 just outside Two Harbors, MN has three lines of "_____ Bay".
