Tennessee comes in with a couple of odd ones: Gruetli-Laager and Soddy-Daisy. Both are fairly near Chattanooga.

There's also Bumpass which is also in Louisa County.

The town of Swayzee, IN, bears a sign claiming to be the only Swayzee in the world. No idea whether or not it's true.

There's a similar sign in Waunakee, Wisconsin. Though it's far from the weirdest name in the state, which I'd personally say is Spread Eagle.
