Here is a road ( University Ave) in Dedham, MA that in a distance defaults into the ramps to and from I-95 NB after passing the overpass.
To the Left isn't any different either. Looks like a typical arterial but is the ramp to I-95 SB with a driveway into the office park just beyond the intersection.
I know in urban areas these are countless when a city street defaults to a freeway ramp, but I'm looking for examples in suburban or rural areas.
Menomonee Falls (suburban Milwaukee) Falls Pkwy turns into the SB on/off ramp for 41/45 at Pilgrim Rd. (Exit 51 A-B)
I immediately thought of Boudinot Drive (https://maps.app.goo.gl/cc85FshmsF9yaN4K6?g_st=ic) here in Fairfax County. It has a 0.2-mile segment connecting Alban and Fullerton Roads, but aside from that, both of its ends are highway ramps connecting to the Fairfax County Parkway at one end and the I-95 HO/T lanes at the other.
Northern end of MA 99 onto US 1 although it's a Jersey freeway.
MA 128 (solo) exit 38. Straight to take 128, turn to stay on the surface road.
I-95 MA exit 90.
I-93 NH exit 1, if it can be considered a surface road. You also have an opportunity to turn there to not get on.

IMO, it is important for safety reasons to incorporate freeway entrance signs in these situations. There should be some warning as to when a normal street suddenly becomes an on-ramp. Of course, in CA and several other (primarily western) states, the freeway entrance sign is used for all on-ramps, but I think a case can be made to at least require it in these types of circumstances, as opposed to run of the mill common interchanges.
The example OP provided is similar to how Broadway in Burlingame used to directly feed into the trumpet ramps for US 101; this configuration was changed significantly to a more conventional overpass setup about 5-6 years ago.

Originally built connecting Almonesson Rd (CR 621) to NJ 41 when the Deptford Mall was built in the 1970s. In the 1980s, it defaulted to NJ 55 when that highway was built. More recently in the 00's, a ramp was added to the other end to connect it into NJ 42 South.
Also to note - Deptford Center Road is only a municipal roadway, but 5 lanes much of its length.

Lawrence Way was built around 1990, while I-680 was being extensively expanded in that area. It's a one-way street whose primary purpose is serving as a nearly half-mile-long on-ramp; it does go through the signal shown in that GSV before turning into an actual on-ramp though. The original on-ramp departed North Main Street much closer to the freeway, and its removal and functional replacement by Lawrence Way cleared some land for commercial development (now a car dealership and a hotel).

I used to wait for the express bus to UC Santa Barbara at that stop, and there was someone who would exit northbound US-101, park right across the street, and then take the bus the rest of the way to campus, presumably to save on parking.

Ridgeview Street at I-44 in Sapulpa, Oklahoma
I used this ramp at about 1 o'clock in the morning to get back on southbound I-44 after having mistakenly taken the old exit 221-B to OK-66. I had no GPS, and was navigating by guesswork until I spotted some signs at the intersection with 96th Street. I followed them to into a seemingly abandoned area, which I found a bit unsettling until I got to the actual ramp.

I should have brought up in the title streets that remain as such until the last moment before it merged. Those in the OP transformed at a traffic signal where the crossroad isn't even another street, but entrances to parking lots.
Onto another location you have MLK Drive in St Petersburg at its north end where it becomes the ramp onto the Howard Frankland Bridge which is I-275.
You do have a Last U Turn sign at the last median break ( as there is no crossroads at the change from street to ramp) but doesn't explain the nature of " Last" meaning. There is curve too beyond the median break so those not familiar with the area won't see the freeway merge until after the curve with no place else to u turn or exit. Once your on I-275 it's 7 miles to exit the first freeway ramp and across the Tampa Bay.
On its north end, State College, PA's Atherton Street northbound turns into ramps for I-99 southbound towards Altoona. (Paradoxical, I know; in reality, both roads travel roughly west.) At the same intersection, Valley Vista Drive northbound intersects Atherton and then defaults into I-99 northbound ramps. On its south end, by which time it's known as Boal Avenue, it turns into ramps for US 322 eastbound. And in the northeastern part of town, Park Avenue eastbound defaults into ramps for I-99 northbound.

For reasons that I will never understand, State College aligns itself around the east-west axis of the one-way pair of College and Beaver Avenues downtown. So Atherton got tagged with the north-south label, and is signed as "North Atherton Street" west of town and "South Atherton" east of it. Similarly, PA 26 - which mostly runs north-south through the area, and is signed as such - is split into "West College Avenue," running south to Pine Grove Mills, and "East College Avenue," running north towards Bellefonte. Makes no sense. Never will...

Especially since it should have been obvious that College and Beaver are north-south by the way the mountains go.

Sorry, not quite. But the I-85 Connector was part of the first section of Interstate to open in North Carolina. The official 1965 map of Mecklenburg County (https://web.lib.unc.edu/nc-maps/interactive/MC_065_1968nm2.php) indicates that Sandy Avenue hadn't yet been constructed. It wasn't until 1971 that a short section of I-85 northeast of there bypassed that original section of Interstate leaving behind this connector. When it opened in 1958, the connector was also the eastern end of Bypass US-29 that followed this original section of Interstate, but US-29 reverted back to its old routing some 5 years after.
By the way, you'll notice that the original routing of the I-85 Connector ended at an interchange with US-29 and NC-49 before the latter was reconfigured to connect with University City Drive. By 1970, that original section of NC-49 had been converted to a fourlane all the way up to the UNC Charlotte campus. I wonder what UNC Charlotte would look like today if it still was directly connected to I-85 like it was originally.
(By the way, 1965 Mecklenburg Map is misidentified as 1968 on the UNC mapping website. But it is really cool how UNC has overlaid a bunch of old maps onto a current GIS mapping system. Click the "Fade Historic Map" checkbox to get a good look at the overlapping features. It doesn't work well for some maps, but this overlay is very impressive).

That is not a suburban street. That is old MA 15, which when it was connected to old CT 15, formed a continuous route through the state and into NYC proper.

The building adjacent to this entrance is ISTHA HQ. This 88EB entrance used to be restricted to ISTHA personnel and was not open to the general motoring public. ISTHA changed a couple of things pavement and signing there and opened it as a publicly available 88EB ramp off US 34/Ogden Ave maybe 10 years ago or so?
