Actually, US 41 will be moved in the Milwaukee area to follow I-41, so I-894/US 45 will be upgrade from a double to quadruple I-41/I-894/US 41/US 45.

Ah that's right; I forgot about that.

Four locations come to mind in Pennsylvania (others can clue me in to any more).
I-99/U.S. 322/U.S. 220
I-376/U.S. 22/U.S. 30
PA 332/PA 413/PA 532
and I think, with the reroute in Lewistown, there is a short triple with U.S. 22/U.S. 322/U.S. 522

I-180/US 15/US 220 in Williamsport

State College, PA: I-99/US 220/PA 26
Meadville, PA: US 6/US 19/US 322
Kittanning, PA: US 422/PA 28/PA 66
Franklin, PA: US 62/US 322/PA 8
Indiana, PA: US 119/PA 56/US 422 Business

This one is questionable depending on the signage in Uniontown, PA:
US 119/US 40/PA Turnpike 43 - http://goo.gl/maps/yF6TM

And there's another one that we'll have in the future if they ever get the gap for I-99 done:
I-80/I-99/US 220

Did some routings get changed in downtown SB?  Last I knew, there were no concurrencies between BUS 20 and BUS 31.  BUS 20 follows Colfax/Lafayette/Lincolnway and BUS 31 follows Michigan/St. Joseph northbound and Main southbound.

No routing change. Because of one-way streets, EB BR-20 cannot turn south on Lafayette Street. Lafayette is one-way north. Instead, 20 continues one block east on LaSalle, and then turns south on Main Street. It's on Main Street that EB BR-20 is concurrent for one block with SB BR-31 and SB SR-933. BR-20 then turns east on Colfax Street.

Traveling in the other direction, WB BR-20 has no concurrency with the other highways.

So this maybe counts as a half-concurrency. I always thought it a little weird that the EB and WB routes cross paths at LaSalle and Lafayette and cross again at Main and Colfax.

I had forgotten that Lafayette was only one-way, so yeah, there is half a concurrency for one block on Main Street.
Indiana: counties 100%, highways 100%
Illinois: counties 100%, highways 61%
Michigan: counties 100%, highways 56%
Wisconsin: counties 86%, highways 23%

I'd have to go back through some of my photos to check, but I think that may be signed with a regular PA 43 sign.
Government would be tolerable if not for politicians and bureaucrats.

This concurrency may not last, if the mayor has his way. He wants to return all downtown streets to two-way and add some roundabouts. This may make sense for some streets, but not the ones that carry SR-933 and BR-31 through downtown. Those are very busy one-way streets that really need the synchronized lights to carry the load. My hope is that INDOT vetoes the idea.

It occurred to me recently that triple concurrencies are relatively rare in Michigan, so I took a stab at listing as many as I can. One likely reason is that compared to a state like Ohio, we just don't have nearly as many numbered highway routes here. Anyone have anything to add to this list, or have an idea of what it would look like for a different state?

US 2/US 41/M-35 (the longest one, from Escanaba to Gladstone)
US 2/US 141/M-95 (Iron Mountain)
US 31/M-37/M-72 (Traverse City)
US 131/M-66/M-72 (Kalkaska)

Here are a few that involve business routes, although I'm not sure if I count business routes for this exercise:

BR I-75/BR US 24/M-59 (Pontiac; west M-59 is concurrent around the loop, but not east M-59)
BR I-94/BR US 127/M-50 (Jackson)
BR I-94/BR US 131/M-43 (Kalamazoo)
BR I-94/M-37/M-96 (Battle Creek)

There are also triplexes (triplices?) that no longer exist as well, due to one or more (or all) of the routes being removed; someone who's more familiar than me with the state map as it has changed over the years might be able to come up with more:

I-75/US 10/US 23 (from Bay City to I-75 exit 93)
I-96/I-69/US 27 (Lansing)
US 23/US 27/US 31 (Mackinaw City)

No longer existent- I69/US27/BL94 at Marshall.

halork

No longer existing: US-41/M-28/M-35 : E of Covington to E of Negaunee.

renegade

Also no longer existing:  I-75/US-10/US-23 between Flint and Standish
Don’t ask me how I know.  Just understand that I do.
