...is that not how it's meant to be? The first exit is on the right of the gantry because it's the closest to that point.

Back to the up arrows signs, I found another on NJ 109 near Cape May, NJ.

Looks like NJDOT is installing these in some locations.  I posted one a few posts up.

I'm pretty sure, based on my reading of the MUTCD, that's what it's supposed to be used for.

I made that clarification because roadman65 called it a "diamond gore warning" in their post about the arrows pointing up. The MUTCD permits that sign to be used in other contexts besides gore points.

2023 MUTCD (underlined emphasis added):

Section 2C.23 Double Arrow Sign (W12-1)
Option:
⁰¹ The Double Arrow (W12-1) sign (see Figure 2C-6) may be used to advise road users that traffic is permitted to pass on either side of an island, obstruction, or gore in the roadway. Traffic separated by this sign may either rejoin or change directions.
Guidance:
⁰² If used on an island, the Double Arrow sign should be mounted near the approach end.
⁰³ If used in front of a pier or obstruction, the Double Arrow sign should be mounted on the face of, or just in front of, the pier or obstruction. Where stripe markings are used on the pier or obstruction, they should be discontinued to leave a 3-inch space around the outside of the Double Arrow sign.
